四种慢性病在三级医疗机构中的费用分析

摘　　要：合理有效地配置医疗资源是医疗管理体制的核心问题。我们对高血压病、冠心病、脑梗死和糖尿病4种慢性病在三级甲等医院、二级甲等医院及社区卫生服务中心诊疗费用的构成情况进行分析,并依据相应指南规范分为指南范围内药费和指南范围外药费。结果显示,大医院对指南执行得较好,社区卫生存在明显的不规范现象。在慢性病的防治中,建议加强卫生主管部门的指导与监督,贯彻落实各类指南,规范慢病防治;加强循证医学研究,找出符合我国国情的安全、有效、经济的药物;创新医疗保险管理机制,做到病种付费与达标控制结合的原则,解决看病难、看病贵以及医疗资源分配严重不公平问题。Medical resource effective allocation was key issue in medical management system. Cost structure of hypertension, coronary heart disease, cerebral infarction and diabetes in tertiary hospital, secondary hospital and primary hospital was analyzed. Based on the medical guideline, cost was divided into in guideline cost and out guideline cost. Tertiary hospital obeyed the guideline well but community health institute was not. In chronic diseases prevention and treatment, government regulation, treatment standardization, evidence based treatment and using safety, effective, economic drug, innovation medical insurance system were considered to solve high cost and in-equity health resource allocation.
