摘 要:目的:研究分析盐酸关附甲素(AHH)在五指山小型猪心肌梗死后慢性心功能异常模型中是否引起血流动力学指标变化。方法:选用五指山小型猪25只,19只予结扎冠状动脉前降支。均合理喂养5~6周后,根据超声心动图指标的变化,将其随机分为心功能正常生理盐水组(n=5)、心功能正常AHH组(n=6)、心功能异常生理盐水组(n=7)及心功能异常AHH组(n=7)。性别、月龄、身长、体重等基线资料无组间差异。分别经Swan-Ganze导管测定应用AHH或生理盐水前后血流动力学指标。应用SPSS11.5软件,采用配对t检验比较各组内用药前后指标变化,采用调整基线资料的协方差比较指标的变化是否存在组间差异。结果:初步动物实验结果证实,心功能异常AHH组与心功能正常AHH组比较,肺动脉舒张压及平均压用药后与用药前差值有所下降(P〈0.01),其余血流动力学指标变化均无统计学意义(P〉0.05);心功能正常模型中应用AHH与生理盐水比较,肺动脉舒张压差值略为升高(P〈0.05)。实验过程中普遍存在心率减慢现象,但与组别关系不大。结论:在慢性心功能异常模型中,AHH对主要血流动力学指标无明显影响。Objective:To analyze the hemodynamic effects of acehytisine hydrochloride (AHH) on chronic post-infarction cardiac insufficiency in Wuzhishan (WZS) micropigs. Methods:Bred properly for 5-6 weeks after coronary artery ligation or false operation, WZS micropigs were randomized into four groups according to their echocardiogram data : normal function saline (NS) group ( n = 5 ), normal function AHH group ( n = 6) ,cardiac insufficiency NS group (n = 7 ) and cardiac insufficiency AHH group (n = 7 ). Surface ECGs and arterial pressure were monitored during the experiments. Hemodynamic data were obtained through Swan-Ganze catheterization at baseline and after AHH (or NS) infusion. SPSS11.5 statistic software package was used for data analysis. Results:The data from the 25 WZS micropigs had on significant differences at baseline. After AHH administration, only slightly raise in diastolic pulmonary arterial pressure was noted in normal function models. Diastolic and mean pulmonary arterial pressures were higher in cardiac insufficiency AHH group after AHH infusion than in normal function AHH group. In chronic cardiac insufficiency models,AHH showed no significant effect on hemodynamic parameters. Reduced heart rate was found in four groups at different time after AHH or NS administration, but no statistical significance between groups. Conclusions:AHH had no obvious adverse effects on hemodynamics in either chronic cardiac insufficiency or normal function models in this experiment.
