经阴道超声测量卵巢形态学参数在诊断PCOS中的价值

摘　　要：目的探讨经阴道超声(TVU)测量卵巢形态学参数在诊断和鉴别诊断多囊卵巢综合征(PCOS)中的临床意义。方法利用TVU测量35例PCOS妇女和35名正常妇女及20例多囊性卵巢(MFO)妇女卵巢表面积(TA)、基质面积(SA)、卵泡计数(NF),测定生殖激素:促黄体生成素(LH)、卵泡刺激素(FSH)、睾酮(T);推算SA/TA和LH/FSH。结果PCOS妇女LH(14.46±4.64)mIu/L、LH/FSH2.36±0.67、T(2.36±0.53)nmol/L显著高于正常对照者(6.03±1.05)mIu/L、1.05±0.08、(1.70±0.45)nmol/L,(tLH=4.14,tLH/FSH=4.72,tT=3.23,均P<0.05)及MFO妇女(6.12±1.03)mIu/L,1.01±0.07,(1.63±0.41)nmol/L(tLH=4.21,tLH/FSH=4.58,tT=3.82,均P<0.05);SA(2.15±0.46)cm2、SA/TA0.43±0.08,显著高于正常对照者(1.17±0.15)cm2、0.28±0.06(tSA=11.98,tSA/TA=8.87,均P<0.01)及MFO妇女(1.20±0.23)cm2、0.26±0.07(tSA=8.62,tSA/TA=7.92,均P<0.05),NF在MFO妇女(13.58±1.75)与正常对照者(7.06±1.22)差异有统计学意义(t=16.23,P<0.05)。相关分析显示:SA/TA与激素相关性良好(rLH=0.75,P<0.01;rLH/FSH=0.71,P<0.01;rT=0.78,P<0.05)。结论SA/TA能有效区别MFO和PCOS,TVU测量SA、TA,计算SA/TA值,为PCOS的诊断和鉴别诊断提供依据。Objective To evaluate transvaginal ultrasonography （TVU） in diagnosis of polycystic ovarian syndrome （PCOS）. Methods The stromal area （SA）,total area （TA） of the ovary, the number of follicles （NF） of 35 cases with PCOS, 20 cases with muhifollicular ovary （MFO） and 35 normal women were detected by TVU. Serum levels of lutenizine hormone （LH）, follicle-stimulating hormone （FSH） and testosterone （T） were measured, and the SA/TA, LH/FSH were calculated. Resuits The serum levels of LH（ 14.46 ± 4.64）mIu/L, T（2.36 ± 0.53 ）nmol/L and LH/FSH （2.36 ± 0.67） in PCOS were higher than those in normal women[（6.03 ± 1.05 ）mIu/L, （ 1.70 ± 0.45 ）nmol/L and （ 1.05 ± 0.08 ）, P〈0.05]and patients with MFO [（6.12 ± 1.03 ）mIu/L, （ 1.63 ± 0.41 ）nmol/L and （ 1.01 ± 0.07）, P〈0.05]. SA （2.15 ± 0.46）cm^2 and SA/TA （0.43 ± 0.08） in PCOS were higher than those in normal women [（ 1.17 ± 0.15 ）cm^2 and （0.28 ± 0.06）, P〈0.01 ] and in patients with MFO （ 1.20 ± 0.23 ）cm^2 and [（0.26±0.07 ）, P〈0.05]. There was significant difference in NF between MFO （13.58 ± 1.75） and norreal women [（7.06 ± 1.22 ）, P〈0.05]. The SA/TA had significant correlations with LH, LH/FSH and T （P〈0.05）. Conclusion TVU is useful in assessment of PCOS.
