摘  要:目的通过比较嗜酸性粒细胞主要碱性蛋白(MBP)和中性粒细胞弹性蛋白酶(NE)在鼻息肉(NP)黏膜上皮、固有层及分泌物中的表达,探讨其在NP发病机制中的作用。方法采用免疫荧光和Axioplan2imaging显微图像分析系统检测20例慢性鼻窦炎患者的NP上皮、固有层及分泌物中MBP和NE的表达,并对检测结果进行t检验、单因素方差分析和Pearson直线相关分析。结果①MBP和NE在上皮组织、固有层和分泌物中的表达均显著高于正常对照组(P<0.05);②MBP在上皮组织和分泌物内的平均阳性表达与NE相比均无统计学差异,而在固有层内的表达有显著性差异(P<0.05);③MBP和NE的平均阳性表达在上皮、固有层和分泌物之间存在显著性差异(P<0.05);④分泌物中MBP、NE以脱颗粒状态为主,而在上皮层和固有层内则主要存在于完整的嗜酸性粒细胞(Eos)和中性粒细胞(Neu)内;⑤上皮脱落区域也有大量MBP、NE表达,而上皮完整区域及固有层内仅有少量MBP和NE表达;⑥Pearson直线相关分析显示NP上皮、固有层、分泌物中MBP和NE的表达均无相关性。结论MBP和NE可能与NP的形成有关。Objective To explore the role in the pathogenesis of nasal polyp (NP) by detecting and comparing the expression of eosinophil major basic protein (MBP) and neutrophil elastase (NE) in nasal NP epithelium, stroma and secretion. Methods Expression of MBP and NE in NP epithelium, stroma and secretion in 20 patients with chronic sinusitis (CS) were detected by immunofluorescence and Axioplan 2 imaging microlmage analysis system, and the results were processed by t test, one-way analysis of variance and Pearson linear correlation analysis. Results The expression of NE and MBP in epithelium, stroma and secretion was significantly different from that of the control group( P 〈 0.05). The was no significant difference between the expression of MBP and NE in epithelium and secretion, while significant difference was found in stroma(P 〈 0.05). There was significant difference in the mean expression of MBP and NE between epithelium, stroma and secretion( P 〈0.05). The MBP and NE in the secretion were mainly found in a degranulated state, while the majority of them existed in the intact eosinophils (Eos) and neutrophils ( Neu). Expression of MBP and NE in large number was also detected in the epithelial shedding region, while less MBP and NE expression was observed in the epithelial intact region and stroma. It was revealed by Pearson linear correlation analysis that the expression of MBP and NE in NP epithelium, stroma and secretion was uncorrelated. Conclusion MBP and NE may play a role in the pathogenesis of NP.
