摘  要:目的观察慢性膝关节炎滑膜组织阿片μ受体的表达。方法以成年人膝关节滑膜组织为研究对象,分为炎性组和对照组,每组25例。炎性组为关节镜下诊断为慢性膝关节炎患者;对照组为关节镜下需行髌骨内固定的髌骨脱位患者。滑膜组织分别在关节镜手术时取自膝关节腔髌上囊、关节前室、髁间窝、关节后室4个部位,采用免疫组化法和RT-PCR技术检测滑膜组织阿片μ受体的表达。结果炎性组膝关节四个部位的滑膜组织阿片μ受体光密度值、阳性细胞百分率、mRNA表达量较对照组显著增高(P<0.05),但各组内膝关节4个部位的滑膜组织阿片μ受体光密度值、阳性细胞百分率、mRNA表达量无明显差异(P>0.05)。结论慢性滑膜炎时膝关节滑膜组织内阿片μ受体表达明显增加,这可以解释膝关节腔内使用阿片类药物缓解疼痛的作用机制。Objective To evaluate the expression of μ opioid receptor of the knee joint synovium tissue in patients with chronic inflammation. Methods The patients were divided into inflammatory group and control group( n = 25 for both). Those who were allocated to the inflammatory group were diagnosed as osteoarthritis by arthroscopy, and in the control group, the patients were having dislocation of patella and took internal fixation with arthroscopy. The synovium tissues were harvested from suprapateller bursa, articulation vestibule, intercondylar fossa and articulation metacele. The synovium were taken to measure μ opioid receptor by immunohistochemistry and RT- PCR in both groups. Results Immunohistochemical and RT-PCR results revealed that the expression of mRNA and the optical density (OD) of immunoreaction of μ-opioid receptor in the knee synovium tissue in the chronic inflammation group were significantly higher than those in the control group (P 〈 0.05). There was no significant difference in the expression of mRNA and the OD of immunoreaction of μ-opioid receptor in the four parts of knee synorium tissue in each group (P 〉 0.05). Conclusion The expression of μ opioid receptor of the knee joint synovium tissue in chronic inflammation was significantly up-regulated. It may play a partial role in the peripheral mechanism of morphine.
