瑞芬太尼和丙泊酚静脉全麻复合硬膜外阻滞在电视胸腔镜手术中的应用

摘　　要：目的评价瑞芬太尼、丙泊酚静脉全麻复合硬膜外阻滞在电视胸腔手术（VATS术）中应用。方法60例电视胸腔镜下行肺手术患者。分为静吸复合全麻组（C）、瑞芬太尼、丙泊酚静脉全麻组（RP）和瑞芬太尼、丙泊酚静脉全麻复合硬膜外阻滞组（RP-EA），每组20例。观察术中血液动力学变化；术毕恢复时间；术后疼痛视觉评分（VAS评分）。结果RP-EA组术中血液动力学稳定性，术毕恢复时间及VAS评分均优于C组和RP组（P〈0.01）。结论瑞芬太尼、丙泊酚静脉全麻复合硬膜外阻滞在胸腔镜手术的应用，术中血液动力学稳定，术后苏醒恢复快，具有安全、有效、可控性强的优点。Objective To appraise the efficacy of extramural anesthesia complex with remifentanil-propofol general anesthesia on the VATS. Methods Sixty patients undergoing VATS were random divided into three groups. Each group contains twenty patients, the group general anesthesia （ group C）, the group remifentanil-propofol intravenous anesthesia （ group RP） and group extradural anesthesia complex with remifentanil-propofol general anesthesia （group RP-EA）. The homodynamic changes during operation and the time to open eyes and pull out the tracheal tube and leave the operating-room were observed. Results During the induction of anesthesia, the VAS was higher and the blood pressure lower and heart rate slower in group RP-EA than those in group C and group RP. Conclusions The extramural anesthesia complex with remifentanil-prepofol general anesthesia on the VATS had the advantage of more stability of homodynamic changes, more controllable, quicker of coming to and shorter time to pull out the tracheal tube.
