腹腔镜手术1467例临床分析

摘　　要：目的：探讨腹腔镜在腹部手术中的应用及并发症的预防。方法：回顾性分析2001年-2006年腹腔镜下胆囊切除、阑尾切除、溃疡病穿孔修补、肠粘连松解、肝囊肿开窗、直肠癌根治术、胆总管囊肿切除术等1467例临床资料。结果：腹腔镜手术应用15种类1467例，其中有28例因胆囊三角解剖不清或胆囊萎缩而中转开腹手术，6例因术中出血而中转开腹。手术并发症5例。结论：腹腔镜手术的适应证随技术进步和设备改善将逐步拓宽，严重并发症发生率可降低。Objective： To study the clinical application of laparoecopic operation and the prevention of the complications.Methods： To review retrospectively 1467 clinical date of laparoscopic cholecystectomy, appendectomy, repair of perforated deer, release of intestinal adhesion,fenestration of hepatic cyst and radical resection of rectal carcinoma, choledechocystectomy was performed between March,2001 and November, 2006. Results： Laparoscopic operation was used in 15 conditions.ln 1467 cases there were 28 cases of conversion into open chelecystectomy because of difficult in Calot＇ s triangle dissection. 6 cases because of hemorrhage. Complicatiom occurred 5 cases included injury of bile duct and hemorrhage. Conclusions： With advanced technology and improvement in medical instrument,the indication of laparoscopic surgery may be broadened. The major complicatiom can be reduced by strict procedures of laparoscopic operation.
