新生儿脑室周围白质软化的危险因素及预后

摘　　要：目的调查5年来新生儿脑室周围白质软化(PVL)发生率、危险因素及预后。方法对2000年12月～2005年11月本院记录资料较完整的806例新生儿中遗留脑性瘫痪61例患儿,至少2次头颅B超或MRI检查,确诊且排除其他疾病的PVL患儿26例为研究组。同期住院的35例无PVL的脑性瘫痪患儿为对照组。行PVL高危多因素回归分析、预后调查。结果PVL26例中早产儿占8.45%,足月儿占1.35%。重度窒息(x3)、孕周(x1)、颅内出血(x14)、低血压(x8)是PVL发生的主要高危因素,其OR值分别为2.843、3.575、3.268、1.947。妊娠高血压综合征、新生儿呼吸窘迫综合征、宫内感染对PVL发生的影响差异也有显著性(Pa<0.05)。结论PVL发生率高,致病因素复杂,预后差,主要后遗症为痉挛性瘫痪及智力低下。　Objective To investigate the incidence rate,risk factors and prognosis of neonatal periventricular leukomalacia（PVL）in recent 5 years in our hospital.Methods Sixty-one cases had the equela of cerebralpalsy among 806 newborns in our hospital from Dec.2000 to Nov.2005,26 cases were research group who were diagnosed by B ultrasonic or MRI at least twice and had been excepted other diseases.Thirty-five cases were control group who were cerebralpalsy patients without PVL and had not significant difference from the research group by statistics.Regression analysis and prognosis investigation of PVL high-risk factors were made.Results PVL were 26 cases and the incidence rate was 3.23%,among which the rate preterm infant was 8.45% and term infant was 1.35%.The severe apnea（x3）,gestational age（x1）,intraventricular hemorrhage（x14）and low blood pressure（x8）were the main high risk factors of PVL,OR of them were 2.843,3.575,3.268 and 1.947 respectively.Pregnancy induced hypertension,neonatal respiratory distress sydrome（NRDS）and intrauterine infection could affect the incidence of PVL and had significant difference（Pa〈0.05）.Conclusions The incidence rate of PVL is high,the factors which can occure PVL are complicated.The prognosis of the PVL is worse,the main equela of PVL were spasticity cerebra palsy and mental retardation.
