利咽解毒方防治鼻咽癌急性放射性口腔黏膜反应临床观察

摘　　要：目的观察利咽解毒方防治鼻咽癌急性放射性口腔黏膜反应的临床疗效及其对鼻咽癌放疗疗效的影响。方法96例患者随机分为治疗组、对照组,各48例。2组均行根治性放疗。治疗组予中药利咽解毒方含服,对照组予西药含漱,2组疗程均从放疗开始至放疗结束。结果治疗组及对照组黏膜反应发生的时间分别为(21±4.2)、(16±3.6)d,放射剂量分别为(40.252±11.758)、(29.625±11.632)Gy,差异有显著性(P<0.05)。2组放射性口腔黏膜反应发生率差异有显著性,治疗组损伤程度明显低于对照组(P<0.01)。2组口腔黏膜反应愈合有效率比较,差异有显著性(P<0.01)。治疗组及对照组总放疗时间分别为(48.78±2.03)、(52.43±3.18)d,差异有显著性(P<0.01)。放疗结束时,治疗组及对照组鼻咽部病灶残留率分别为10.42%、14.58%,颈部淋巴结转移灶残留率分别为34.29%、38.24%,差异无显著性。结论利咽解毒方能延迟鼻咽癌放疗患者黏膜反应发生的时间,提高其发生的剂量,降低反应级别,促进口腔黏膜反应愈合,对放疗效果无明显影响。Objective To evaluate the clinical effect of Liyanjiedu decoction for the prophylaxis and treatment of acute radiotherapy-induced oral mucositis of nasopharyngeal carcinoma （NPC） and the influence to the effect of the radiotherapy to NPC. Methods A total of 96 patients with NPC were randomly divided into treatment group and control group, 48 cases for each group. The two groups were given radical radiotherapy. The treatment group was given Liyanjiedu decoction for buccal. The control group were given western medicine for mouthwash. The course of two groups was all from the beginning of radiotherapy to the end of it.Reanlts The occurring time of radiotherapy-induced mucositis in treatment group and control group were （21± 4.2）d and （16± 3.6）d, and radiotherapeutic dose were （40.252±11.758）Gy and （29.625± 11.632）Gy, with significant difference between the tow groups （P〈0.05）. The occurrence rate of radio-induced mucositis had significant difference between the two groups. The injury of mucosa in treatment group was less than control group （P〈0.01）. The effective rate of oral mucosa healing of the treatment group was much better than control group （P〈0.01）. The total radiotherapeutic time was （48.78±2.03）d in treatment group and （52.43±3.18）d in control group （P〈0.01）. The residual rate of pharynx nasalis after radiotherapy in treatment group and control group was 10.42% and 14.58%, and the residual rate of lymph node in cervical part was 34.29% and 38.24%. There was no significant difference between the two groups. Liyanjiedu decoction can delay the time and increase the dose of occurring radiotherapy-induced mucositis of nasopharyngeal carcinoma, cut down the grade of radio-induced mucositis, enhance the healing of oral mucosa. It had no influence on the effect of radiotherapy.
