糖尿病血瘀证与超敏C反应蛋白及糖化血红蛋白的关系

摘　　要：目的观察糖尿病血瘀证患者的血清超敏C反应蛋白(hsCRP)及糖化血红蛋白(HbA1c)的水平,探讨其与糖尿病血瘀证的关系。方法糖尿病血瘀证患者58例及非血瘀证患者65例,均测量身高、体重,计算体重指数(BMI),同时测空腹血糖(FPG)、血脂、hsCRP及HbA1c,并测24h尿微量白蛋白排泄率(24h UAER)。结果血瘀证组FPG、低密度脂蛋白胆固醇(LDL-C)、三酰甘油(TG)、HbA1c、hsCRP较非血瘀证组明显升高,且差异显著(P<0.05或0.01);高密度脂蛋白胆固醇(HDL-C)低于非血瘀证组,差异显著(P<0.01);24h UAER及总胆固醇(TC)水平高于非血瘀证组,但无显著性差异(P>0.05)。直线相关分析表明,血瘀证组hsCRP与HbA1c、24h UAER、BMI、LDL-C、TG、TC均正相关,但排除病程、BMI、血脂等因素影响,逐步回归分析结果显示hsCRP与HbA1c独立相关。结论长期高血糖和炎症反应可能是糖尿病血瘀证形成的重要原因之一,hsCRP和HbA1c作为炎症反应和高血糖的特异指标,联合检测将为糖尿病血瘀证的判断提供新的依据。Objective To study the level of serum high-sensitive C-reactive protein and HbAlc in diabetes patients with blood stasis syndrome, and to investigate the relationship among them. Methods 58 cases of diabetes patients with blood stasis syndrome and 65 cases without blood stasis syndrome were observed, The level of FPG, hsCRP, HbAlc, 24 h UAER, TC, TG, HDL-C and LDL-C were estimated. Results The level of FPG, LDL-C, TG, HbAle, hsCRP of diabetes with blood stasis syndrome were higher than the control group （P〈0.05 or 0.01）, HDL-C was lower than the control group （P〈0.01）, and the level of TC and 24 h UAER were also higher than control group, but it was not statistical meaning （P〉0.05）. In patients with blood stasis syndrome, the hsCRP was positively correlated to HbAlc, 24 h UAER, BMI, LDL-C, TC, TG, but the multifactor regression analysis indicated that hsCRP was only correlated to the HbAlc independently. Conclusions The long-term hyperglycemia and inflammatory may be one of the reasons for the formation of blood stasis syndrome of diabetes patients, hsCRP and HbAlc may be new evidences for diagnosis of diabetes patients with blood stasis syndrome.
