针刀治疗寰枢关节紊乱引起颈性眩晕的临床疗效观察

摘　　要：目的:通过针刀治疗与西药对照治疗寰枢关节紊乱,观察针刀治疗寰枢关节紊乱的临床疗效,寻求治疗寰枢关节紊乱的有效方法。方法:通过针刀松解术松解寰枢关节周围软组织的痉挛、挛缩,配合针刀医学手法以针刀医学特有的寰枢关节紊乱临床分型方法为依据整复寰枢关节紊乱,并与西药组进行随机对照,以临床症状、体征改善程度及X线张口位片及椎动脉多普勒超声检查来评估其疗效。结果:45例寰枢关节紊乱患者,随机分为针刀治疗组和西药对照组,用针刀治疗21例,其中治愈10例,显效5例,好转3例,无效3例,治愈率为47.7%,显效率为23.8%,有效率为85.8%;西药对照组24例,其中治愈1例,显效2例,好转5例,无效16例,治愈率为4.2%,显效率为8.4%,有效率为33.4%,说明针刀治疗的临床疗效明显优于西药治疗(P<0.01)。治疗前后的寰枢关节的寰椎与齿突轴线差值及齿突与侧块间距差值具有显著差距(P<0.01),椎动脉多普勒超声检查显示针刀治疗前后椎动脉血流动力学变化显著,具有统计学意义(P<0.05)。结论:认为针刀是治疗此病的比较有效的方法,与西药对照组进行比较具有很显著的优越性。Objective：To compare the clinical effects of treating atlanto-axial joint disorder by acupotome and drugs, and to explore the effective method for atlanto-axial joint disorder. Method： The peripheral issue was dissoluted by th acopotomy and manipulated according to the special typing of acupotomy, and clinical random comparison. The effect was evaluated by the relief degree of the clinical symptom, the signs, the mouth-open projection and the vertebral doppler alcohol. Results： Fouty-five cases were divided to the acupotomy group and the drug group at random. Twenty-one cases were treated by acupotomy of which 10 cases were cured, 5 cases were effective, 3 cases were improved, 3 cases were inefficative. After treatment, the cured rate was 47.7%, the effectual rate was 23.8% and the total effective rate was 85.8%. Twenty-four cases were treated by drug, of which 1 cases were cured, 2 cases were effective, 5 cases were improved, 16 cases were inefficarive. After the treatment, the cured rate was 4.2%, the effectual rate was 8.4% and the total effective rate was 33.4%. It proved that the effect of acupotomy was better than that of drug. （P〈0.01） The deviation of the atlas alignment from that of the dens of axis was decreased significantly （P〈0.01）. The vertebral doppler alcohol showed that the vertebral hemodynamic changed significantly. Conclusion：The experiment means that acupotomy is more effective than drugs for this disease treatment.
