立体定向置管引流术治疗少量高血压脑出血的临床研究

摘　　要：目的:探讨立体定向抽吸引流术治疗少量高血压脑出血的近期和远期治疗效果,并对手术相关问题进行讨论。方法:本研究选取自2005年2月至2006年12月在我院神经内科及神经外科住院的160例少量高血压脑出血病人为研究对象,根据治疗方法的不同分为两组,即立体定向抽吸引流术组和非手术治疗组。立体定向手术组68例,非手术治疗组92例。分别统计两组病人的近期疗效和远期的生活质量并进行对比。近期观察指标为住院期间患者的意识状态、患肢肌力;出院3个月后进行随访,观察指标为日常生活能力分级(ADL)。结果:近期观察患者意识状态、患肢肌力指标显示手术组与非手术组存在差异(P<0.05),远期观察指标显示两组病例ADL评分有差异(P<0.05)。结论:少量高血压脑出血病人立体定向抽吸引流术的近期和远期总体疗效优于非手术治疗,因此对于有明显神经功能缺失的患者应行手术治疗。Objective： Hypertensive intracerbral hemorrhage （ICH） was one of the most lethal stroke types. The aim of the study was to compare the short-term and long-term therapeutic efficacy of stereotactic surgery for slight hypertensive intrace, rebral hemorrhage with that of medical treatment, and discuss the related problems about stereotactic surgery. Methods： We chose the 160 cases with slight hypertensive cerebral hemorrhage treated in our hospital from February 2005 to December 2006. The cases were divided into two groups according to the difference of treatment methods. 68 cases took stereotactic operation, and 92 cases took only drug treatment. Besides taking antibiotics, other drug treatments of the operating group were similar to the non-operating group. We analyzed and contrasted the short-term therapeutic effect and long-term living quality of the two groups. We observed the following indexes when the two groups treated in hospital： conscious state, animal force. Three months after discharge, we evaluated the ADL grade of the two groups. Results： There were statistical differences of conscious state and animal force between the two groups, the operating group had better results than non-operating group. The long-term index indicated that there was statistical difference of ADL grades between the two groups, the operating group had better results than non,operating group. Conclusion： The total therapeutic effect of stereotactic surgery was better than drug treatment in the treat： ment of slight hypertensive cerebral hemorrhage. The cases with obvious loss of nerve function should take stereotactic operation.
