土黄连液灌注治疗乳腺导管扩张伴炎症53例临床研究

摘　　要：目的研究乳管内视镜下乳头溢液导管腔内结构、病变性状。探索治疗炎症性乳头溢液的新方法。方法应用乳管内视镜对乳头溢液患者进行检查,选择其中101例诊断为导管扩张及导管炎症的良性病变患者,随机分2组,治疗组53例经病变乳孔灌注土黄连液2 mL,对照组48例经病变乳孔灌注庆大霉素80000 U治疗,每周灌注1次,连续灌注8次,随访3个月后乳管内视镜复查。结果灌注治疗后随访3个月,治疗组50例停止溢液,总有效率94.34%;对照组总有效率81.25%;土黄连液组疗效优于常规庆大霉素对照组(P<0.05)。结论土黄连液局部药物灌注治疗乳腺导管扩张伴炎症性乳头溢液疗效较好。Objective To study the intraductal structure and pathological changes of mamillary polyrrhea by ductoscope, and to investigate a new therapeutic method for treating inflammatory mamillary polyrrhea. Methods The patients with mamillary polyrrhea were examined by the ductoscope and then 101 among them diagnosed as mammary duct ectasia with duct inflammation were chosen and randomly divided into 2 groups. The treatment group （n = 53 ） was treated with Tuhuanglian Solution （2 mL） by perfusion through ill mammary foramen and the control group （n = 48 ）, with gentamicin （80 000 U）, once a week and for total 8 times. After the follow-up visits for 3 months the patients were given the reexaminations by the ductoscope. Results After 3 months there were 50 cases in the treatment group cured. The total effective rate was 94.34 % in the treatment group and 81.25 % in the control group. The curative effect of the treatmen group was better than that of the control group （ P 〈 0.05 ） by statistical analysis. Conclusion Tuhuanglian Solution has a good curative effect through local perfusion on mamillary polyrrhea induced by mammary duct etasia accopanied by inflammation.
