摘  要:目的探讨阿魏酸钠(Sodium Ferulate,SF)预处理对心内直视手术的心肌保护作用。方法40例二尖瓣置换病人随机分为两组:对照组和SF预处理给药组,每组20例。对照组常规行二尖瓣置换术;SF预处理给药组病人在术前第5天开始,口服阿魏酸钠片3 mg/(kg.d),2次/d,连服5 d;分别在转流前、升主动脉开放后30 min及术后6、12、244、8 h抽取静脉血标本,采用全自动生化分析仪定量分析法测定cTnI、CK、CK-MB的活性;采用分光光度计比色法测定SOD含量;采用硫代巴比妥酸显色法测定MDA含量。记录体外循环辅助时间,术后正性肌力药物用量。结果与对照组术后各相应时间点比较,术后6、12、24、48 h SF预处理给药组的cTnI、CK、CK-MB的活性均明显降低(P均<0.01,P<0.05),SOD活性均显著升高(P均<0.05,P均<0.01),MDA含量均显著降低(P均<0.05,P均<0.01),体外循环辅助时间缩短(P<0.05),术后多巴胺用量减少(P<0.01)。结论SF预处理对心内直视手术有较好的心肌保护作用。Objective To investigate the cardioprotective effect of sodium ferulate (SF) in open heart operation. Methods Forty patients undergoing mitral valve replacement were respectively selected and randomly divided into control group(n = 20)and SF pretreatment group (n = 20). In SF pretreatment group,SF was taken(3 mg/kg/d, Bid) orally for five days preoperatively. The changes of lactate cardiac troponin I(cTnI), creatine kinase(CK), isoenzymes of creatine kinase (CK-MB), superoxide dismutase(SOD) and malonaldehyde (MDA)levels in the serum were measured before operation,and at 30 min,6 h,12 h, 24 h,and 48 h after operation. The time of assist extracorporeal circulation and the dosage of inotropic agent after operation were measured. Results In SF pretreatment group,the myocardial enzyme of the patients were decreased significantly(P〈0.05 or P〈0.01) ;The ability of anti-oxygen free radicals were increased significantly (P〈0.05 or P〈0.01) ;The time of assist extracorporeal circulation were shorten(P〈0.05)and the dosage of using inotropic agent after operation were lower(P〈0.01). Conclusion The patients undergoing open heart surgery can get better myocardial protective effect in SF pretreatment group.
