体外循环下不阻断升主动脉小儿心内直视术60例临床分析

摘  要:目的:评价体外循环下不阻断升主动脉行心内直视术对小儿心肌保护作用。方法:60例小儿先天性心脏病,均在浅低温心脏跳动下完成手术,回顾性分析60例患者的临床资料。结果:全组转机时间最长为58 min,最短为29 min,平均转机(36.86±21.14)min。全组无1例手术死亡。所有患者均手术后2~3 h清醒、术后3~10 h撤离呼吸机,全组无脑气栓和传导束损伤发生。患者1 d后拔出引流管可以自由活动,术后住院3~6 d出院时心功能均恢复至Ⅰ级。结论:不阻断升主动脉,浅低温体外循环心脏不停跳下心内直视术用于小儿先天性心脏病矫治,心肌保护安全有效,临床效果满意。Objective To evaluate the effect of no aortic lamping on myocardiac protection under cardiopulmonary bypass(CPB) during open heart surgery.Methods:2006,60 infants with congenital heart malformations underwent open heart surgery using the technique of no aorta-clamping under CPB.They were 35 boys and 25 girls.Their age mean(4.65±1.34)years.Their weight mean(15.61±3.72)kg.The congenital heart malformations included:ventricular septal defects in 12,atrial septal defects in 30,trilogy of Fallot 3,pulmonary stenosis in 8 and PDA in 7.0 pen hear surgery was performed under mild hypothermic CPB with beating heart in 60 patients.Results:No cerebral embolism was found ang no deaths.Conclusion:Open heart surgery without aortic-cross clamping in infants is afeasible,effective and safe technique for myocardial protection during CPB.
