摘  要:目的探讨慢性压力超负荷兔左心室结构和功能的动态改变。方法采用肾上腹主动脉缩窄方法制备兔左心室短期压力超负荷模型,设立假手术组,术后2周和8周通过心导管法测定左心室功能,然后测量左心室质量指标;采用胶原酶两步消化法分离获取左心室中层单个心肌细胞,应用激光扫描共聚焦显微镜技术测定中层细胞体积和蛋白含量。结果术后2周和8周,缩窄组主动脉收缩压、舒张压、左室收缩压均较假手术组显著升高;术后2周,与假手术组比较,缩窄组左室压力最大上升和下降速率(±dp/dtm ax)、左室舒张末压(LVEDP)、左室收缩指数、T值(左室松弛时间常数)无明显改变,左室舒张末期容积(LVEDV)、左室质量指数(LVW I)、左室后壁厚度(PWT)及中层细胞体积、蛋白含量亦无明显差异;术后8周,与假手术组比较,缩窄组±dp/dtm ax、左室收缩指数及LVEDV无明显改变,LVEDP和T值明显升高,LVW I、PWT及中层细胞体积、蛋白含量显著增加。结论腹主动脉缩窄2周,兔左心室收缩功能正常,此时并无心肌肥大,缩窄8周时左室收缩功能正常而舒张功能明显受损,且左心室显著肥厚。Objective To explore the dynamic changes of left ventricular structure and function of rabbit undergoing chronic pressure overload. Methods Healthy adult New Zealand rabbits were divided into sham operation group and operation group. Rabbits in operation group received partial ligation of superrenal aortic aorta to induce the left ventricular pressure overload. In 2 and 8 weeks after surgery, hemodynamic and heart weight parameters were measured. At the same time, single rabbit ventricular myocytes were isolated from M layers of LV using the two-phase enzymatic dissociation methods. Myocyte volume and protein content per cell were measured by confocal laser scanning microscope. Results In 2 and 8 weeks after surgery, the aortic arterial systolic blood pressure (SBP), diastolic blood pressure (DBP) , LV systolic pressure(LVSP) increased significantly comparing with sham operation group. In 2 weeks, comparing with sham operation group, there were no significant changes in ± dp/dtmax, left ventricular end diastolic pressure (LVEDP) , LV systolic index (LVSI) , T ( diastolic time constant) and left ventricular end diastolic volume (LVEDV) , left ventricular weight index (LVWI) , posterior wall thickness, midmyocardial myocyte volume, protein content in operation group. In 8 weeks, there were no significant changes in ± dp/dtmax, LVSI, LVEDV between sham operation group and operation groups, however, LVEDP,T, midmyocardial myocyte volume and protein content in operation group were markedly increased than sham operation group. Conclusion In 2 weeks after abdominal aortic artery ligation,left ventricular systolic function in rabbit can remain normal , however, no left ventricular hypertrophy occurred at this time. The mechanism to maintain the normal systolic function needs further study.
