有限切开复位交锁髓内钉治疗股骨干粉碎骨折的临床研究

摘　　要：目的：研究采用有限切开复住交锁髓内钉治疗股骨干粉碎骨折的临床价值及方法。方法：采用有限小切口切开复位，改进髓内钉进钉点、有限扩髓，交瞄准器的带锁髓内钉固定，部分植骨或对开放骨折抗生素搅拌植骨填补骨缺损，以及术后早期CPM功能锻炼等方法，治疗各类股骨干粉碎骨折120例，其中闭合骨折92例，开放骨折28例。结果：120例获得随访，随访期6-36个月，平均18个月。闭合骨折平均愈合时间16周（平均10周～34周）；开放骨折平均愈合时间20周（范围14周～36周），5例延迟愈合。术后6个月骨折愈合率为90％，术后1年骨折愈合率为100％，无断钉、感染等并发症。术后综合疗效评定均为优良，临床效果满意。结论：有限切开复位，交锁髓内钉是治疗股骨干粉碎骨折的简便、可靠、安全和有效的方法。Objective： To study the clinical effects of the treatment on comminuted femural fracture with interlocking intramedullary nail in minimally invasive incision. Method： 120 cases of comminuted femural fracture treated with interlocking intramedullarynail. 92 cases were closed fractures and 28 cases were open fractures. Minimal invasive incision, limited narrow cavity rasping, improved the placement of nail, bone allografting or the graft bone mixed with antibiotics in open fracture for bone defect as well as early training with CPM after operations were used in treating. Result： All patients were followed up for average 18 months （range 6-36 months）. The closed fractures had got union within average 16 weeks （range 10 -34 weeks） and open fractures 20 weeks （range 14 -36 weeks）. 5 cases had got delayed union. 91% of the cases had got union within 6 months and all within one year. Threr were no infection and nail breakage. All the cases had got excellence result. Conclusion： The comminuted femural fracture treated with interlocking intramed-diary nail in minimally invasive incision is a simple, reliable, safe and effective technic.
