摘 要:目的探讨免疫共刺激分子CD40/CD40L在过敏性紫癜患儿外周血单个核细胞(PBMC)中的表达及其意义。方法研究对象为30例过敏性紫癜(HSP)患儿,其中第一组(12例)为紫癜性肾炎(HSPN),第二组(18例)为过敏性紫癜无肾受累组(NO-HSPN);20例正常儿童作为对照组。应用逆转录-聚合酶链式反应(RT-PCR),并采用植物血凝素(PHA)和地塞米松(Dex)进行干预,从转录水平检测PBMC中CD40和CD40L的表达。结果与正常对照组比较,HSP患儿CD40L mRNA的表达明显增高,HSPN组CD40L mRNA的表达高于NO-HSPN组,但CD40 mRNA的表达差异无统计学意义;加入PHA后,三组CD40L mRNA表达均明显增高,尤以HSPN组增加更明显,加入抗-CD40LmAb后基本恢复正常,而加入PHA后,三组CD40 mRNA的表达略有增高,与受刺激前相比差异无统计学意义;加入Dex后,过敏性紫癜患儿两组CD40L mRNA表达均明显减少,尤以HSPN组减少更明显,对照组的表达减少不明显,而三组CD40 mRNA的表达减少均不明显。结论过敏性紫癜患儿PBMC共刺激分子CD40L的表达异常增高,表明CD40L可能参与HSP的发病。Objectives To investigate the expression of co-stimulatory molecules CD40 and CIMOL in patients with Henǒch-Schǒnlein purpura (HSP) and to explore their possible roles in the disease. Methods Thirty (30) HSP patients were enrolled and were divided into two groups, HSPN group (with nephritis, n = 12) and NO-HSPN group (without nephritis, n = 18), respectively. Control group included 20 healthy children. Reverse transcription-polymerase chain reaction technique was applied together with phytohematoagglutinin and dexamethasone as intervention, to quantify CD40 mRNA and CD40L mRNA expression in peripheral blood mononuclear cells. Results Compared with controls, the expression of CD40L mRNA increased more in HSP patients, especially in HSPN group. However, CD40 mRNA expression had no significant increase. The CD40L mRNA expression in peripheral blood mononuclear cells of all groups, especially in HSPN, increased obviously after phytohematoagglutinin being added and then decreased to normal after anti-CD40L monoclonal antibody being added. More decreased CD40L mRNA expression were found in HSP, especially in HSPN group after dexamethasone being added, while CD40 mRNA expression of all three groups decreased little. Conclusions The abnormal expression of CD40L may play a pathological role in the development of HSP.
