摘 要:目的比较Parkland公式复苏与改良液体复苏对犬弹烧复合伤海水浸泡后血气及酸碱平衡的影响。方法成功建立海水浸泡弹烧复合伤模型后,依模型将20只成年杂种犬随机分为海水浸泡弹烧复合伤组(对照组,n=8)、标准Parkland公式复苏组(标准组,n=6)与改良液体复苏组(改良组,n =6),动态监测伤前、伤后4、7、10、20、28h血气及酸碱水平的变化。结果Parkland公式复苏在一定程度上改善了浸泡后代谢性酸中毒,但改善作用弱、持续时间短;改良组纠正酸中毒作用稳定、持久。结论以Parkland公式为参照进行常规复苏时有一定的局限性,不适合该类复合伤的早期救治;而以Parkland公式复苏为基础的改良方案具有较好的复苏价值。Objective To compare effects of resuscitation with the Parkland formula or the improved protocol on arterial blood-gas and acid-base balance in projectile-burn combined wound in dogs with seawater immersion. Methods Created model of projectile-burn combined wound in dogs with seawater immersion, 20 dogs were randomly divided into three groups:projectile-burn combined wound with seawater immersion (control group,n = 8 ) , Parkland formula resuscitation ( standard resuscitation group, n = 6 ) and improved protocol ( improved group,n = 6). The levels of arterial blood-gas and acid-base balance before injure as well as at 4,7, 10,20 and 28 h after injure were recorded. Results After resuscitation with the improved protocol,changes of arterial blood-gas and acid-base balance ameliorated better than those after resuscitation with Parkland formula. Conclusions There are some limitations in conventional recovery according with Parkland formula, which is not suitable for such combined wound in the early treatment, while the improved protocol based on Parkland formula would be better value.
