左氧氟沙星联合化疗和DOTS方案对复治肺结核的比较

摘　　要：目的观察左氧氟沙星联合化疗治疗复治肺结核的临床效果。方法将60例复治肺结核患者随机分为2组,30例采用左氧氟沙星联合化疗为治疗组,30例采用标准DOTS方案治疗为对照组。治疗1年间,留取两组患者痰液行结核分枝杆菌涂片检查,并观察两组3、6、12个月的痰菌阴转率和12个月时的X线吸收好转率。结果治疗组3、6、12个月患者痰菌阴转率分别是53.3%、75.3%、83.3%,对照组3、6、12个月痰菌阴转率分别是20.0%、30.0%、40.0%;两组12个月X线吸收好转率分别为86.6%、46.6%。两组比较,痰菌阴转率及X线吸收好转率差异均有显著性(P<0.01)。结论左氧氟沙星联合化疗治疗复治肺结核有明显优势。Objective To observe the effect of levofloxacin combined with chemotherapy on retreated pulmonary tuberculosis. Methods 60 cases of retreated pulmonary tuberculosis were randomly divided into two groups. 30 cases of treatment group received levofloxacin combined with chemotherapy; 30 cases of control group received DOTS. After a year of treatment, mycobacterium tuberculosis from sputum of the patients were collected and then analyzed to evaluate the negative conversion rate of sputum after3,6 and 12 months treatment and X-ray improvements rate after 12 months treatment. Results In treatment group, the sputum negative conversion rates in3,6 and 12 months were 53.3%,73.3%,83.3%,but in control group the sputum negative conversion rates in3,6 and 12 months were 20.0%,30.0%,40.0% respectively. There was significant difference between two groups for X-ray improvements after 12 months treatment （P 〈 0.01 ）. Conclusion Levofloxacin combined with chemotherapy is effective in the treatment of retreated pulmonary tuberculosis.
