摘 要:目的观察同种异体心肌细胞移植对心肌梗死大鼠心肌的修复作用和心功能的影响。方法选取110只雄性SD大鼠用于制作心肌梗死模型,术后2周共存活53只,死亡57只。从53只建模成功的SD大鼠中随机选取30只分为3组(每组10只):心肌细胞移植组(A组)、培养液基质注射组(B组)和单纯心肌梗死组(C组)。A组将原代培养的乳鼠心肌细胞移植到心肌梗死区和梗死边缘区;B组采用与A组同样方法在心肌梗死区和梗死边缘区注射细胞培养液基质;C组不做任何处理。2周后观察A组移植细胞存活情况。采用Langendorff装置检测各组左心室收缩压(LVSP)、左心室舒张末压(LVEDP)、左心室内压最大上升速率(+dp/dtmax)、左心室内压最大下降速率(-dp/dtmax)和左心室做功(LVW)。结果A组与B组及C组相比,LVSP分别从(47.9±4.6)mmHg和(47.6±3.9)mmHg升高到(52.0±3.0)mmHg(P<0.05或P<0.01);LVEDP分别从(18.8±2.7)mmHg和(24.7±5.8)mmHg降低到(15.8±1.5)mmHg(P<0.05或P<0.01);+dp/dtmax分别从(1 900±198)mmHg/s和(1 737±347)mmHg/s升高到(2 260±337)mmHg/s(P<0.05)。3组间-dp/dtmax和LVW比较差异无显著性(P>0.05)。A组大鼠的心肌中均发现移入的乳鼠心肌细胞,经5-溴脱氧尿核苷(BrdU)标记的免疫组化染色和抗心肌肌钙蛋白I抗体(anti-cTn I)标记的免疫组化染色证明移入的细胞是心肌细胞。结论异体移植的乳鼠心肌细胞可在心肌梗死区存活,并能改善心功能,尤其是改善左心室收缩功能。Objective To study the effects of transplanted allogenic cardiomyocytes on the repair and function of infarcted myocardium of rat. Methods 110 male rats were operated on to establish models of myocardial infarction (MI), and 2 weeks later, only 53 of them were found having survived the procedure. 30 out of the 53 established rat MI models were allocated to 3 groups (n = 10 each) for varied experimental purposes: group A, to receive cultured allogenic cardiomyocytes from neonate rat (0.25 ml of culture medium containing 5×10^9 cells/L) injected into the site of infarction and its adjacent area ; group B, to receive a blank injection into the infarct zone ; group C, to serve as the MI control. The results were examined 2 weeks after the injection by determining the left ventricular function of the isolated rat heart mounted on the Langendorff apparatus and by making immunohistologic studies to confirm the taking of the grafted cells. Results The left ventricular systolic pressure (LVSP) was found increased from (47.9±4.6) mmHg and (47.6±3.9) mmHg in groups B and C to (52.0±3.0) mmHg in group A ( P 〈 0.05 or P 〈 0.01 ). The left ventricular end diastolic pressure (LVEDP) was found decreased from ( 18.8±2.7) mmHg and ( 24.7±5.8 ) mmHg to ( 15.8±1.5 ) mmHg ( P 〈 0.05 or P 〈 0.01 ). The maximum rate of intraventrieular pressure rise ( + dp/dtmax ) was increased from ( 1900±198) mmHg/s and (1737±347) mmHg/s to (2260±337) mmHg/s (P 〈 0.05 ). Immunohistology showed living neonatal cardiomyocytes were present in all of the 10 rats of group A: BrdU labeling showed the grafted cells were alive, the positivity of anti - cTn I indicated that the BrdU labeled cells belong to the type of cardiomyocyte. Conclusions Neonatal cardiomyocytes can survive the transplantation and improve heart function significantly, especially that of the left ventricle.
