摘 要:目的探讨不同氧流量对急性肺损伤患者痰液性状的影响。方法将120例急性肺损伤患者随机分为A、B、C三组,分别以4L/min、6L/min、8L/min的氧流量经鼻导管给氧,比较各组患者给氧前、给氧第1、2、3天痰液的黏性、弹性和给氧前、给氧3dPO2等临床指标的变化。结果A、B组给氧第1、2、3天痰液黏性、弹性显著低于给氧前(均P<0.05),C组给氧第1、2天显著低于给氧前,第3天显著高于给氧前(均P<0.05);三组给氧不同时段痰液黏性、弹性比较,差异有显著性意义(均P<0.05),C组处于较高水平。三组给氧3dPO2、HR、R显著优于给氧前(均P<0.05),B、C组显著优于A组(均P<0.05),B、C组比较,仅R1项差异有显著性意义(P<0.05)。结论对于急性肺损伤患者,6L/min的氧流量比较合理,既可达到有效的氧疗,又不使痰液过度黏稠。 Objective To examine the effect of different oxygen flow rate on the sputum viscoelasticity of the patient with acute lung injury.Methods A total of 120 patients with acute lung injury were randomly divided into 3 groups and received oxygen via nasal catheter at a rate of 4 L/min,6 L/min,8 L/min(designated as group A,B and C respectively).The changes in sputum viscoelasticity and PO2 were observed before and after the oxygen therapy.Results The viscoelasticity of the sputum in group A and B 1,2 and 3 day(s) after the oxygen therapy was significantly lower than that before the therapy(P〈0.05 for all).In group C,the sputum viscoelasticity 1 and 2 day(s) after the oxygen treatment was significantly lower than that before the treatment(P〈0.05 for both),however,on the 3rd day,it was significantly higher than that before the treatment(P〈0.05).There were significant differences in the viscosity and elasticity at different time points among the 3 groups(P〈0.05 for all) and the viscosity and elasticity of sputum in group C were relatively higher.In the 3 groups,the results in PO2,heart rate,respiratory rate on the 3rd day were better than those before the oxygen administration(P〈0.05 for all),with the results in group B and C being significantly better than those of group A(P〈0.05 for all).Comparison between group B and group C showed that significant difference existed only in respiratory rate(P〈0.05).Conclusion For patients with acute lung injury,the flow rate of 6 L/min of oxygen is optimal because it can achieve better effect and will not results in sticky sputum.
