中西药结合治疗腹腔感染大鼠血浆肿瘤坏死因子-α和白细胞介素-6的变化

摘  要:目的:观察抗生素联合中药治疗对腹腔感染时血浆炎症因子水平的影响,为治疗腹腔感染提供新策略。方法:选取雄性SD大鼠100只,随机分为假手术组、模型组、泰能组(泰能20mg/kg)、血必净组(血必净8ml/kg)、联合用药组(泰能20mg/kg+血必净8ml/kg)5个组,每组20只。采用盲肠结扎穿孔诱发腹腔感染模型,从尾静脉注入不同药物,给药后2h、6h从股静脉抽取静脉血2·5ml,离心后测定血清肿瘤坏死因子-α(TNF-α)及白细胞介素-6(IL-6)浓度。结果:模型组血浆TNF-α水平2h较6h高;泰能组与模型组比较,2h较低(P<0·01),6h无显著性差异(P>0·05);血必净组和联合用药组明显低于模型组和泰能组(P<0·01)。模型组血浆IL-6水平明显高于假手术组(P<0·01),各治疗组均较模型组低(P<0·01),而联合用药组又较血必净注射液或泰能组更低(P<0·01)。结论:联合应用中药血必净和西药泰能治疗腹腔感染,能更有效地降低血浆TNF-α、IL-6水平,是治疗腹腔感染的新策略。Objective: To investigate the change in plasma levels of pro-inflammatory mediators during the treatment of intra-peritoneal infection with traditional Chinese medicine and antibiotics simultaneously in rat,in order to find a new treatment strategy of intra-peritoneal infection. Method: One hundred male SD rats were divided into five groups randomly: sham group, control group, Imipenem group ( 20 mg/kg), Xuebijing ( 血必净) group (8 ml/kg) ,combination group(Imipenem 20 mg/kg+Xuebijing 8 ml/kg), with 20 rats in each group. Cecum ligation and puncture (CLP) was done to produce intra-peritoneal infection . Each group was further divided into two sub-groups, the treatment was begun either 2 hours or 6 hours after CLP, 2.5 ml of blood were taken from the femoral vein. The plasma levels of tumor necrosis factor-α(TNF-α)and interleukin-6 (IL-6)were measured after centrifugation. Results:Plasma TNF-α level in control group peaked 2 hours after CLP and decreased at 6 hours. It was lower in antibiotics group than in control group (P〈0. 01), and there was no difference 6 hours after operation (P〈0.05). In Xuebijing group and combination group TNF-α levels were significantly lower than in control group and antibiotic group (P〈0. 01). Plasma IL-6 level in control group was significantly higher than in sham group (P〈0.01). Plasma IL-6 levels were lower in all the other groups compared with control group, and that in combinationgroup was lower than in antibiotics or Xuebijing group (P〈0.01). Conclusion: The combination of Xuebijing and antibiotics (Imipenem) for the treatment of intra-peritoneal infection could decrease the plasma levels of TNF-α and IL-6.
