摘  要:目的通过检测HBV携带者的血清、前列腺液和精液中乙型肝炎病毒(HBV)-DNA含量,研究三者的关系及其临床意义。方法选择经血清学检测诊断为男性HBV携带者120例,采用荧光定量聚合酶链反应(FQ—PCR)技术测定其血清、前列腺液和精液中HBV—DNA含量,并对所有检测指标进行相关性分析。结果120例携带HBV男性中,HBsAg、HBeAg、HBcAb三项阳性者的血清、前列腺液和精液中HBV—DNA的阳性检出率均为100.0%;HBsAg、HBeAb、HBcAb三项阳性组中阳性率分别为86.0%、83.7%和83.7%;HBsAg、HBcAb两项阳性组中的阳性率分别为87.2%、82.1%和84.6%。精液HBV—DNA含量与血清HBV—DNA含量呈正相关(r=0.896),且血清HBV—DNA含量高于精液含量10倍左右:前列腺液HBV—DNA含量与血清HBV—DNA含量呈正相关(r=0.893),且血清HBV-DNA含量高于前列腺液含量10倍左右;前列腺液HBV—DNA含量与精液HBV—DNA含量呈正相关(r=0.993),且前列腺液HBV—DNA含量与精液含量比较差异无统计学意义。结论精液、前列腺液与血清三者中HBV—DNA含量呈正相关。血清HBV—DNA阳性的育龄男性应做精液、前列腺液HBV—DNA检测,以对其阳性者采取防治措施,其性伴侣也应加强防护并进行预防接种。Objective To examine the hepatitis B virus (HBV)-DNA content in serum, prostatic fluid and semen in patients with chronic HBV and study the relationships with them and their clinical significance. Methods Serum,prostatic fluid and semen HBV-DNA of 120 cases HBV infection were detected by fluorescent quantitative polymerase chain reaction(FQ-PCR) and analyzed the relationships. Resuits In the 120 cases, the HBV-DNA positive rate of HBsAg, HBeAg, HBcAb positive samples in serum, semen and prostatic fluid were all 100.0%. The HBV-DNA positive rate of HBsAg, HBeAb,HBcAb positive samples in serum, semen and prostatic fluid were 86.0%, 83.7% and 83.7%. The HBV-DNA positive rate of HBsAg, HBcAb positive samples in serum,semen and prostatic fluid were 87.2% ,84.6% and 82.1%. The amount of HBV-DNA in serum and in semem had positive relationship (r = 0.896) and the amount of HBV-DNA in serum was higher than that in semen about 10 times. The amount of HBV-DNA in prostatic fluid and in serum had the positive relationship(r = 0.893 ) and the amount of HBV-DNA in serum was higher than that in prostatic fluid about 10 times .The amount of HBV-DNA in semen and in prostatic fluid had the positive relationship(r = 0.993 ) and the amount of HBV-DNA in prostatic fluid and in semen had no significant difference. Conclusions The amount of HBV-DNA in semen,prostatic fluid and.serum has the positive relationships. The male whose HBV-DNA is positive in serum must be detect the amount of HBV-DNA in semen or prostatic fluid and do something to defend. His sex-companion should be inoculated with a HBV vaccine.
