摘  要:目的探讨1型糖尿病(T1DM)患者中甲状腺自身免疫发生情况。方法对104例T1DM、100例2型糖尿病(T2DM)患者和100例健康对照(NC)者进行甲状腺过氧化物酶抗体(TPO-Ab)、甲状腺球蛋白抗体(TG-Ab)检测,T1DM和T2DM患者还检测谷氨酸脱羧酶抗体(GAD-Ab)。结果(1)T1DM患者任一甲状腺自身抗体(TAA)的检出率为25.0%(26/104),高于T2DM患者的12.0%(12/100)和NC者8%(8/100),(P<0.05)。(2)GAD-Ab阳性的T1DM患者TAA的检出率为38.6%(17/44),高于GAD-Ab阴性组的18.3%(11/60)(P<0.01)。(3)女性T1DM患者的TPO-Ab、TG-Ab的检出率分别为30.4%(14/46)、28.3%(13/46),均高于男性的15.5%(9/58)和10.3%(6/58)(P<0.05)。结论GAD-Ab阳性和女性T1MD患者易合并甲状腺自身免疫紊乱。Objective To evaluate the prevalence of thyroid autoimmunity in type 1 diabetics. (T1DM) Methods The frequencies of thyroid peroxidase antibody (TPO-Ab) and thyroglobulin antibody (TG-Ab) were determined in 304 subjects, including 104 T1DM, 100 type 2 diabetics (T2DM) and 100 controls. Glutamic acid decarboxylase antibody (GAD-Ab) was measured in T1DM and T2DM. Results (1) Thyroid autoantibody (TPO-Ab or TG-Ab) existed more frequently in T1DM (26/104,26.0%) than in T2DM (12/100,12.0%) and controls (8/100,8.0%) (P 〈0.05). (2) The prevalence of thyroid autoim- munity was increased in T1DM with GAD-Ab than those without it (38.6% vs 18.3% ). (3)The frequencies of TPO-Ab and TG-Ab were 30.4% (14/46)and 28.3% (13/46)respectively in female T1DM, which were higher than in male 15.5%(9/58)and lO. 3%(6/58)respectively(P 〈0.05). Conclusion GAD- Ab positive and female in T1DM are the higher risks for thyroid autoimmunity.
