护理干预对老年肥胖高血压患者糖代谢的影响

摘　　要：目的探讨护理干预对伴糖耐量异常的肥胖高血压病患者的影响。方法2003年起在我院门诊就诊的高血压且伴糖耐量异常的老年患者176例，分为干预组80例和对照组96例，经3～3．5年的护理干预及跟踪随访，将两组患者基本状况、临床生化指标、依从性及新发生糖尿病和心血管病的发生情况进行比较。结果干预组的依从性均明显高于对照组（P〈0．01），在体重指数、腰围、餐后2h血糖及胰岛素、胰岛素抵抗指数方面均较对照组明显降低（P〈0．01）；干预组2型糖尿病和心血管病的发生率也明显低于对照组。结论有效的护理干预对伴糖耐量异常的肥胖高血压病患者有着显著作用，有利于病情的控制，防止其进一步的发展，并能有效是降低2型糖尿疾病和心血管病的发生。Objective To discuss the effect of nursing intervention on glucose metabolism in elderly obesity hypertensive patients. Methods One hundred and seventy-six elclerly obesity hypertensive patients were olevided into intervention group （ n = 80） and control group （ n = 96）. To give nursing intervention and compliance follow-up investigation for 3 to 3.5 years on the intervention group, and compare it with the control group. Results There are differences on weight index, waist line, systolic pressure, diastolic pressure, FPG, 2 hPG, FING, 2 hPI, insulin, insulin counteraction index and glycerinate in both groups before and after follow-up investigation （P 〈 0.05 ）. Compared with the control group, there are obviously differences on weight index, waist line, 2 hPG, insulin and insulin counteraction index in the intervention group （P 〈 0.01）, and there are prominent differences on the ratio of taking medicine on time, diet therapy, exercise therapy, seeing doctors again timely, having self-conscious of checking blood glucose and pressure （ P 〈 0.01 ） .Condusion By giving nursing intervention to the elderly obesity hypertensive patients with glucose intolerance abnormity, the incidence of 2-Diabetes Mellitus can be deceased and the occurrence of cardiovascular suddenness can be reduced. Nursing intervention is importount to the patients＇ condition
