摘  要:本研究联合应用常规经胸超声(TTE)和经食管超声(TEE),对不同类型主动脉夹层(DeBakeyⅠ型夹层12例及Ⅲ型夹层11例)之不同部位主动脉膨胀性进行对比研究。用TTE测量主动脉瓣上方3cm处升主动脉收缩期和舒张期内径,TEE测量降主动脉上段近峡部处收缩期和舒张期期内径。同时测量静息状态下右上肢肢动脉血压。主动脉膨胀性用简化的Young氏公式求得。结果显示主动脉夹层升主动脉和降主动脉膨胀性均明显低于正常对照组。且Ⅰ型主动脉夹层是升主动脉膨胀性下降更明显,而Ⅲ型主动脉夹层降主动脉膨胀性降低更甚,其主要病变部位与主动脉夹层原发病变部位正好吻合。The purpose of the present investigation was to study the distensibility of both ascending and descend-ing aorta in different types of aortic dissection by combined transthoracic and transesophageal echocardiography. Aortic dis-tensibility was measured at the ascending aorta (3 cm above the aortic valve) and tbe descending aorta (distal to the left sub-clavian artery) from the changes in echocardiographic diameters and the pulse pressure in 12 patients with DeBakey type Ⅰaortic dissection, 11 patients with DeBakey type Ⅲ aortic dissection, 12 age-and gender-matched normal control subjectswere also measured. Aortic distensibility was calculated using the Youth formulas: 2× (pulsatile change in aorticdiameter) / (diastolic aortic diameter) / (aortic pulse pressure)- The present study demonstrated that the distensibility ofboth the ascending and the descending aorta is decreased in aortic dissection. The more interesting findings were that themost severe parts of damage were accorded with the primary tearing in both DeBakey type Ⅰ and type Ⅲ dissection fromthe view of aortic distensibility.
