摘 要:目的 对比研究不同剂量卡维地洛和美托洛尔对异丙肾上腺素(ISO)所致心力衰竭大鼠左心室结构重构和功能重构的影响。方法 雄性Wistar大鼠135只。随机分为对照组和实验组。实验组ISO 340mg/kg皮下注射。24h后重复1次,8周后将实验组68只存活的大鼠再随机分为:ISO组、大剂量卡维地洛组、小剂量卡维地洛组、大剂量美托洛尔组、小剂量美托洛尔组。8周后分别进行心脏超声、血流动力学检查。结果 与对照组比较.ISO组大鼠左心室舒张末期内径、左心室收缩末期内径、左心室舒张末期压、体质量、心室质量、左心室质量、左心室质量与体质量比值均明显增加.而左心室射血分数、左心室短轴缩短率、左心室收缩压、左心室内压最大上升和下降速率均明显降低(P〈0.05)。与ISO组比较,卡维地洛和美托洛尔不同剂量组大鼠心脏功能都能显著改善。卡维地洛大剂量组疗效优于小剂量组(q=4.28,P〈0.05),同时优于大剂量美托洛尔组(q=4.33,P〈0.05);美托洛尔大剂量组优于小剂量组,但差异无统计学意义(q=3.87,P〉0.05),卡维地洛和美托洛尔在小剂量组间差异无统计学意义(q=3.91。P〉0.05)。结论 不同剂量卡维地洛和美托洛尔都能有效抑制ISO所致心力衰竭大鼠左心室结构和功能重构。而且,剂量越大疗效越明显,卡维地洛优于美托洛尔。Objective To compare the effects of carvedilol and metoprolol at different doses in preventing hearts of rats from structural and functional remodeling. Methods 135 male Wistar rats were randomly assigned to control group and isoproterreol (ISO) group, the latter was injected twice with ISO at a 24 hour-intervaL 8 weeks afterwards 85 surviving rats were randomized into ISO group, low carvedilol (LCAR, 10 mg·kg^-1·d^-1), high carvedilol(HCAR, 30 mg·kg^-1-d-1), low metoprolol(LMET, 20 mg·kg^-1-d^-1) and high metoprolol(HMET, 100 mg·kg^-1-d^-1) group. Following 8 weeks of drugs therapy, echocardiogram, hemodynamics and pathology examinations were performed. Results Compared with control group, left ventricular end-diastolic dimension(LVEDD), left ventricular end-systolic dimension (LVESD), heart rate(HR), left ventricular end-diastolic pressure(LVEDP), body weight(BW), ventricular weight(VW), left ventricular weight (LVW) and the ratio of LVW/BW were all increased significantly, while left ventricular ejection fraction (LVEF), left ventricular fractional shortening (LVFS), left ventricular systolic pressure (LVSP) and the maximal rate of rising and descending( ± dp/dtmax) of LV pressure were all significantly decreased(P 〈 0.05). In comparison with ISO group, the LVEDD, LVESD, LVEDP, BW, VW, LVW and the ratio of LVW/BW were all decreased significantly, while the LVEF, LVFS, LVSP and ± dp/dtmax were all significantly increased in both carvedilol and metoprolol group at different doses. HCAR group were improved significantly compared with LCAR and HMET groups (q = 4.28 or 4.33, P 〈 0.05), and there was no significant difference between HMET and LMET groups(q = 3.87, P 〉 0.05 ), as well as between LCAR and LMET groups (q = 3.91, P 〉 0.05).
