2004年青海省14例肺鼠疫病例的临床资料分析

摘　　要：目的 分析肺鼠疫的临床特点，提高对肺鼠疫的诊断和治疗水平。方法 对2004年青海省囊谦县1起肺鼠疫爆发流行的14例肺鼠疫病例，按鼠疫诊断标准（GB15991—1995）确定诊断，分析14例肺鼠疫病例的临床资料。结果 6例鼠疫死亡病例中，原发性败血型鼠疫继发肺鼠疫1例，原发性肺鼠疫继发败血型鼠疫4例，原发性肺鼠疫1例。8例现症鼠疫病例中，原发性肺鼠疫并发感染性休克2例，原发性肺鼠疫合并慢性肺源性心脏病、心功能不全、心力衰竭Ⅲ度、多发性浆膜腔积液1例，原发性肺鼠疫4例，感染后自己治愈1例（间接血凝阳性，滴度为1：5120）。8例现症鼠疫病例经控制感染、抗休克、强心、利尿和对症支持治疗痊愈。结论 2004年青海省囊谦县鼠疫爆发流行确诊肺鼠疫14例，治愈8例，死亡6例；在8例现症鼠疫病例中，均具有典型的肺鼠疫临床症状。Objective To summarize the clinical characters of lung plague in order to improve diagnosis and treatment for plague. Method Clinical data of 14 human plague cases in one outbreak of plague in Niangqian County of Qinghai Province in 2004 were analyzed. National Plague Diagnosis Standard（GB 15991-1995） was used to confirmed the diagnoses followed by the analysis of cyclical data. Result Among 6 died cases, 1 case was diagnosed as primary septicemia plague and secondary lung plague （occupying 7.14% of the total）, 4 cases of primary lung plague were transformed into septicemia plague （accounting for 28.57% ）, leaving 1 case as primary lung plague （occupying 7.14%）. Among other 8 cured patients, 2 cases were diagnosed as primary lung plague ihtegrated infective shock （accounting 14.29% of the total）, 1 case was primary lung plague later accompanied with syndromes of chronic pulmonary heart disease, cardiac insufficiency, heart failure of III degree and multiple dropsy of serous cavity （occupying 7.14%）, 4 cases were primary lung plague （occupying 28.57%）, 1 case was self-cured （occupying 7.14%, IHA positive at the titer of 1 ： 5120）. These 8 patients recovered their healthy after going through control of infection, management of anti-shock, cardiotonic and diuresis and expectant treatment. Conclusion 14 cases of lung plague have been confirmed in this plague outbreak with 8 cases cured and 6 cases died. The 8 plague cases All have had typical manifestations of lung plague.
