摘 要:目的:在分子水平上探讨人脑外伤后病理生理变化机制,为临床正确评估患者伤情及预后提供理论依据.方法:采用免疫组织化学技术和多功能真彩色病理图像分析系统(CMIAS),对31例颅脑外伤患者局部脑损伤区和12例无脑损伤猝死患者脑标本相同脑区神经细胞中C-fos基因的表达进行了检测.结果:脑伤患者局部伤区及周围神经细胞中C-fos表达较无脑伤尸检对照组有明显增高并具时程特点.GCS<8分组高于GCS≥8分组,两组周内死亡组高于存活组,差异显著.结论:局部受损脑组织中C-fos基因的表达在一定程度上可作为评估颅脑伤情、预后的临床参考指标之一.Aim: To explore the pathologic and pathophysiologic mechanism following traumatic brain injury at moleculor level, and to provide some theoretical guidance for the correct clinical evaluation of the patients' traumatic condition and for prognosis. Methods: C fos gene expression in the local neural tissue was studied in 31 cases of brain injury and in 12 autopsy cases of sudden death without brain injury, adopting immunohistochemical techniques and the colorous multi function image analysis system (CMIAS). Results: In all the brain injury cases, there was a dramatic increase in C fos gene expression at the site of injury and in surrounding tissues and furthermore followed by a specific time course, compared with that in autopsy cases. Significantly higher level of C fos expression was found in the injured neural tissue of the GCS<8 group than the GCS≥8 group, and also in that of the non survivors within two weeks post trauma than the survivors. Conclusion: It suggests that C fos gene expression can, to an extent, act as one of the clinical indicators of extent of brain injury and prognosis.
