摘　　要：目的 观察应用鸦胆子油乳联合阿霉素治疗难治性恶性胸腔积液的临床疗效和不良反应。方法 对41例临床难治性恶性胸腔积液进行胸腔引流并行腔内化疗，经2～3d引流干净全部胸水后经导管注入鸦胆子油乳60—100ml，阿霉素40—60mg，封管同时处理相关不良反应。7—9d1次，按疗效重复引流给药，1月后评价疗效和相关不良反应。结果 41例患者中可评价疗效39例，其中CR11例，PR21例，有效率82．1％（32／39），主要不良反应包括轻微胸痛、胃肠反应等。结论 鸦胆子油乳联合阿霉素治疗难治性恶性胸腔积液有明显疗效，不良反应患者可以耐受，值得临床推广应用。Objective To evaluate the efficacy of Brucea javanica oil emulsion（BJOE） combined with Adramycin（ADM） in patients with refractory malignant pleural effusion（RMPE）. Methods The chest cavity liquid of 41 patients with RMPE were drawn out by putting central venous catheter,and a dose of 60 to 100 ml BJOE plus 40 to 60 mg ADM was injected,once every 5 to 7 days;about 2 time for a course of treatment,then the effects and the side effects were observed in and after one month. Results The effective rate was 82.1% （ 32/39 ）, The side effects were thoracalgia （ n = 3 ） and mild digestive sickness （ n = 1 ）. Conclusions The effect of B JOE combined with ADM is good for the patients with RMPE and the side effect is also mild. The method should be highly recommended in the future clinical treatment.
