摘 要:目的研究低剂量安体舒通对肥厚心室肌电生理特性的影响。方法将30只纯种日本大耳白兔随机分为安体舒通组、环缩组和对照组各10只,安体舒通组和环缩组环缩腹主动脉构建左心室肥厚模型,对照组仅游离腹主动脉(不结扎)。术后第2天安体舒通组喂服安体舒通10mg/kg,对照组和环缩组喂服安慰剂;连续8周后停药1d。用心脏电刺激单相动作电位记录技术测定心室MAP时程(MAPD_(90))和心室有效不应期(VERP),用免疫印迹方法测定反向钠-钙交换(NCX)蛋白表达。结果环缩组和安体舒通组心脏重量/体质量明显高于对照组,快速刺激后VERP明显长于对照组,NCX表达水平均显著高于对照组,P均<0.05;VERP延长时间(ΔVERP)与NCX表达水平明显相关(r=0.68)。结论单用低剂量安体舒通不能明显延缓心肌肥厚的发展,对肥厚心室肌频率相关性电重构亦无明显影响;此可能与其不能下调肥厚心室肌NCX高表达有关。[ Objective] To determine whether aldactone alone can have an impact on the electrical remodeling in the hypertrophied myocardium. Methods Rabbits were randomized as three group: constricted group( n = 8) , control group (n = 7 ) and constricted + aldactone group( n = 6). Left ventricular hypertrophy was induced in rabbits by abdominal aortic constriction, Sham-operated control rabbits underwent the same operation, except that the arota was not constricted. One day after surgery, the constricted and sham-operated animals were receive vehicle for a total duration of 8 weeks, while the constricted + aldactone rabbits were given aldactone ( 10 mg/kg) for the same duration. Eight weeks after feeding, the electrophysiological parameters were recorded by the monophasic action potential recording technique in three sites including right ventricular outflow tract (RVOT) , left ventricular outflow tract (LVOT) and left ventricular apex (LVA). The myocardial Na^+ -Ca^2+ exchanger protein was semiquantitated by western-blot analysis. Results Compared with the sham-operated control rabbits, the heart-to-bedy weight ratio was increased significantly in the constricted rabbits and constricted + aldactone rabbits, 2.9 ±0. 31 versus 2.20 ±0.05 ( P 〈 0. 05) and 2.83 ± 0.32 versus 2.20 ± 0.05 ( P 〈 0. 05 ), the latter two groups were not different significantly. Compared with the control rabbits, 90% of monophasic action potential duration after short-time rapid right atrial stimulation in the three sites (RVOT, LVOT and LVA) was not different significantly in the constricted rabbits and constricted + aldactone rabbits, effective refractory period after rapid stimulation increased significantly in the left ventricular site of the latter two groups, ( 18±6)ms versus (8±4)ms and ( 16±8)ms versus (8±4)ms in the LVOT site, (20±6)ms versus (8±5)ms and( 17±7)ms versus (8±5)ms,but the increase was not different significantly in the latte
