摘  要:目的:探讨桡神经深支自发断裂的解剖学因素,为临床诊断该病及选择手术入路及方法提供理论依据。方法:成人上肢标本16侧,沿臂部桡神经走行进行大体及显微解剖。结果:(1)18.9%的桡神经主干及深、浅支存在"翻转现象";(2)桡侧返血管与桡神经深支存在3种毗邻关系,可对其造成卡压、夹持作用;(3)37.5%的桡神经深支在进入Frohse弓近端(3.1±1.2)cm处表面横跨一异常纤维束带;(4)桡侧腕短伸肌腱性部分与Frohse弓存在3种位置关系,可在不同平面对神经产生压迫作用。结论:(1)桡神经"翻转现象"是桡神经深支自发断裂的形态学基础及潜在易发因素;(2)而桡侧返动脉、异常纤维束带、桡侧腕短伸肌腱性结构及Frohse弓是其发病的解剖学基础(继发因素)。Objective: To study anatomic factors concerning with the spontaneous disruption of the deep branch of radial nerve, and provide basis for clinical diagnosis, treatment and the selection of operation approach. Methods: 16 adult fresh cadaverous upper extremity specimens were included in this study, which were dissected following the course of radial nerve by gross anatomy and microdissection. Results: (1) In 18.9 % specimens, the phenomenon of "turnover" presented for the trunk, deep and superficial branches of radial nerve; (2) Three relationship concerned with the recurrent branch of radial arteries and the deep branches of radial nerve,which might be the reasons of the crush and clamp on radial nerve; (3) In 37.5% specimens, a abnormal fiber bundle acrossed the deep branch of radial nerve before it left the proximal end of Frohse arcade about 3.1 ± 1.2cm; (4) 3 types of relationships between the origin of the ECRB and the arcade of Frohse caused nerve oppression at the different planes. Conclusions: (1) The phenomenon of "turuover" in the radial nerve is the morphologic basis and latent etiological factor of the spontaneous disruption of the deep branch of radial nerve; (2) The recurrent branch of radial arteries, abnormal fiber bundles, the origin of the tendon structures of the ECRB and the arcade of Frohse are anatomic factors of spontaneous disruption.
