摘  要:目的:比较颈椎后路单、双开门手术后的三维稳定性及抗疲劳性,为临床术式选择提供生物力学依据。方法:新鲜成人颈椎标本12例,模拟后路单、双开门椎管扩大术式,随机分2组,每组6例,A组行单开门,椎板开口间距为5mm;B组行双开门,棘突开口间距为5mm。标本进行生物力学测试,获得三维稳定性和抗疲劳性数据。统计方法采用独立样本t检验及配对样本t检验。结果:单开门术式组手术前、后与双开门术式组手术前、后颈椎的活动范围比较无显著性差异(P>0.05)。单、双开门术式在疲劳测试前后各个方向的运动范围无显著性差异(P>0.05)。结论:颈椎后路单、双开门术式的三维稳定性及抗疲劳性无差异,临床上可根据术者的熟练程度选择。Objective: To compare biomechanical characteristics between two different laminoplasties of cervical spine for clinical choice. Methods: 12 fresh cadaveric spine specimens were harvested and divided into 2 groups averagely. Samples of group A were performed one-door laminoplasty with the distance between vertebral planes of 5mm, as well group B performed two-door laminoplasty with the distance between spinal processes of 5mm. The data of the stability and antifatigue, including bending, extension, lateral bending and rotation, were collected and calculated. Results: There were no significant difference of ROM before and after operation between one-door and two-door laminoplasy groups (P〉0. 05), as well ROM before and after fatigue test between one-door and two-door laminoplasty groups (P〉0.05). Conclusions: The stability and antifatigue between one-door or two-door laminoplasty of cervical spine have no difference. It is important for selecting different operations according to clinical experience of operators.
