超声心动图在先天性心脏病封堵术围手术期中应用价值的探讨

摘　　要：目的：探讨超声心动图在先天性心脏病封堵术围手术期中的应用价值。方法：应用超声心动图筛选55例适合封堵术的先天性心脏病患者,观察周缘情况,测量分流孔径及周边的大小;监测封堵过程,评价封堵效果。结果：超声心动图测量房间隔缺损最大径6-36mm（平均19.4±6.69mm）,选择封堵器10-42mm;室间隔缺损最大径4-10.5mm（平均6.5±2.71mm）,选择封堵器6-12mm;动脉导管最大径3-14.3mm（平均6.98±3.18mm）,选择封堵器4-20mm。术后复查封堵器固定良好,无移位;房间隔缺损患者两心室缩小,室间隔缺损患者左房缩小,动脉导管未闭患者左室、左房均缩小。结论：超声心动图能准确测量分流孔径的大小,指导选择封堵器型号,进行术中监测,及时反映封堵术后心脏结构及功能的改善情况,在封堵术中具有重要的应用价值。Objective To evaluate the applied values of ultrasonic cardiogram in congenital cardiopathy before the operation.Method 55 cases of congenital cardiopathy were studied by ultrasonic cardiogram,and they all fitted the occlusion.Then we observed its rim,measured its size,detected the occlusion and evaluated the effect.Results The size of atrial septal defect（ASD） measured by ultrasonic cardiogram was 6-36mm（（19.4±6.69）mm）,the size of selected occluder was 10-42 mm.The size of ventricular septal defect（VSD） was 4-10.5mm（（6.5±2.71）mm）,the size of selected occluder was 6-12mm.The size of patent ductus arteriosus（PDA） was 3-14.3mm（（6.98±3.18）mm）,the size of selected occluder was 4-20mm.Recheck the position of occluder good after occlusion.Left ventricle and right ventricle of ASD patients deflated,left atrium of VSD patients deflated and left ventricle and left atrium of PDA patients deflated.Conclusion ultrasoil cardiogram can measure the size of defect exactly,guide the size of selected occluder,detect the occlusion and feedback condition of the heart in time after the occlusion.Ultrasonic cardiogram acts as an important role in the occlusion.
