摘 要:目的探讨慢性乙型肝炎(乙肝)患者外周血单个核细胞(PBMC)凋亡率在乙肝病毒(HBV)感染慢性化机制中的意义及凋亡PBMC的半胱氨酸基-天冬氨酸特异性蛋白酶9(Caspase-9)活性在PBMC凋亡过程中的意义。方法采用流式细胞仪检测25例慢性乙肝患者及15例健康对照者PBMC的凋亡率,并用比色法检测凋亡PBMC内Caspase-9的活性。结果慢性乙肝患者HBV DNA阳性组PBMC凋亡率及PBMC内Caspase-9活性均高于HBV DNA阴性组(均P<0.01)及健康对照组(均P<0.01),HBV DNA阴性组上述两指标均高于健康对照组(分别P<0.05;P<0.01)。慢性乙肝患者PBMC中Caspase-9活性与其凋亡率呈正相关(P<0.01)。结论慢性乙肝患者PBMC存在凋亡现象,可能为乙肝慢性化机制之一;其中的Caspase-9活性增高,提示Caspase-9可能参与了慢性乙肝患者PBMC的凋亡过程。Objective To evaluate the significance of apoptosis of peripheral blood momonuclear cells(PBMC) in the pathogenensis of chronic HBV infection by detecting the apoptotic rates of PBMC and the significance of Caspase-9 in the apoptosis of PBMC by detecting its activity. Methods The apoptotic rates of PBMC in 25 chronic hepatitis B(CHB) patients and 15 healthy control were assayed by flow cytometry and Caspase-9 activities were detected by eolorimetrie methods respectively. Results The apoptotic rates and Caspase-9 activity in PBMC of HBV DNA positive CHB patients were respectively higher than those of HBV DNA negative CHB patients (both P〈 0. 01) and normal controls group (both P〈0. 01), and HBV DNA negative CHB patients were higher than those of normal controls group (P〈0. 05;P〈0. 01). Positive correlation between Caspase-9 activities and apoptosis levels of PBMC in CHB patients was obvious (P〈0. 01). Conclusion Apoptosis of PBMC in the patients with chronic hepatitis B is found, which maybe one of the pathogenesis of chronic hepatitis B. The activity of Caspase-9 increased in the above -mentioned process, suggesting that Caspase-9 may be involved in this process.
