心力衰竭治疗前后血浆白细胞介素-18的变化及临床研究

摘　　要：目的探讨不同级别心功能衰竭(CHF)患者治疗前、后血浆白细胞介素-18(IL-18)水平的临床意义。方法采用酶联免疫法(ELASA)检测116例CHF患者血浆IL-18水平,并以31例健康者作对照,分析CHF患者治疗前、后心功能分级与血浆IL-18水平的变化。结果CHF患者血浆IL-18水平显著高于对照组[(570.99±63.18)pg/ml和(201.82±46.09)pg/ml,(595.24±74.33)pg/ml和(201.82±46.09)pg/ml,(657.07±73.86)pg/ml和(201.82±46.09)pg/ml,F=6.68,P<0.01]。经过治疗心功能改善后血浆IL-18水平显著下降(t=4.56,5.99,3.22,3.78,P<0.01)。结论IL-18可能参与CHF的发生及发展过程,监测血浆水平对判断心衰、指导治疗、评价疗效有一定参考价值。　Objective To measure the changes of the levels of plasma interleukin -18（IL-18） in the patients with congestive heart failure （CHF） of different degree and to study its clinical significance. Methods The levels of plasma IL-18 in 116 patients with CHF were measured by enzyme linked immune sandwich assay （ELISA）,31 healthy person as control group. The changes of the levels of plasma IL-18 in patients with CHF before and after treatment, and the relationship of the levels of plasma IL-18 in the patients with different degree of CHF were analysed. Results The results showed that the levels of plasma IL-18 in the patients with CHF were significantly higher than those in normal controls [（570.99±63.18）pg/ml vs（201.82±46.09）pg/ml,（595.24±74.33）pg/ml vs （201.82±46.09）pg/ml,（657.07±73.86）pg/ml vs（201.82±46.09）pg/ml,F=6.68,P〈0.01）]. The levels of plasma IL-18 in the effective patients with CHF after treatment were lower than that before treatment （t=4.56,5.99,3.22,3.78,P〈0.01）. Conclusion IL-18 may involve in the pathogenesis and development of heart failure,so the monitoring of the cytokines may be helpful for the evaluation of heart failure condition.
