牵拉代替缝合处理小伤口效果分析

摘　　要：目的探讨胶条牵拉代替缝合处理小伤口的方式在基层医疗单位使用的安全性及可行性。方法67例受伤24h内,伤口长1～4cm,伤口深达皮下、创缘较整齐、较清洁的患者中31例(牵拉组)采用胶条牵拉创伤部位,36例(清创组,受伤不超过8h)采取清创缝合,观察两组效果,并进行对比分析。结果牵拉组中29例伤口甲级愈合,2例乙级愈合,伤口平整。清创组中31例伤口甲级愈合,3例乙级愈合,2例丙级愈合,伤口均显"蜈蚣脚"状瘢痕。两组愈合情况间差别无显著性意义(χ2=1.09,P>0.05)。结论牵拉与清创缝合相比,伤口愈合情况相似。胶条牵拉法处理小伤口,受伤8h以上仍能应用,操作简便,不增加创面的附加损伤,能够粘合裂开的伤口,利于伤口愈合,便于基层医生使用。Objective To inquiry into the safety and the possibility of the way in which the adhesive tape is used to pull small wound in place of suturing in fundamental medical institutions. Methods In 67 cases of wound hurting within 24h, 31 cases whose length of wound is 1～4 cm, depth of wound gets to the subcutaneous part, and the wound margin is tidy and clean were treated by pulling the wound with adhesive tape （ pulling group）, and 36 case of wound hurting less than 8h were treated by debridement and suturing （suturing group）. The results of the two groups were comparatively analyzed. Results In the pulling group, 29 cases were A intention, 2 case B intention, with neat healing of the wound. In the suturing group, 31 cases were primary intention, 3 cases secondary intention, and 2 cases tertiary intention, but all of which had a centipede shape scars. There was no significant difference in healing condition between the two groups （ X^2 =1.09, P〉0.05）. Conclusion The healing conditions of the pulling and suturing are similar. The adhesive tape pulling is good for small wound. It is still suitable for the wound hurting for more than 8h. It is simple for operation, does not increase the additional injury to the wound, can make the ruptured wound bound so as to benefit the healing of the wound, being suitable for doctors in the fundamental medical institutions to utilize.
