活血化淤消癥通络中药治疗糖尿病肾病76例临床研究

摘　　要：目的观察活血化淤消癥通络中药治疗糖尿病肾病(DN)的临床疗效。方法将76例DN患者随机分为治疗组(39例)和对照组(37例),两组均予常规降糖治疗。治疗组予活血化淤消癥通络胶囊,口服4粒/次,3次/d;对照组予贝那普利,口服10mg/次,1次/d。疗程12个月。检测尿微量白蛋白排泄率(UAER)、血β2-微球蛋白(β2-MG)、尿β2-MG、尿α1-微球蛋白(α1-MG)、D-二聚体(D-D)、血浆纤维蛋白原(FIB)。结果两组患者治疗前UAER、血β2-MG、尿β2-MG、尿α1-MG、FIB、D-D水平间差异无显著性意义(P>0.05);治疗后两组UAER、血β2-MG、尿β2-MG、尿α1-MG、FIB、D-D水平间差异有统计学意义(P<0.05);治疗组治疗后FIB、D-D水平与治疗前比较,差异有统计学意义(P<0.05)。结论活血化淤消癥通络中药治疗DN,在减少尿蛋白,改善肾功能和高凝状态方面明显优于贝那普利。Objective To observe the clinical effects of blood - flow - promoting herbs in the treatment of diabetic nephropathy （DN）. Methods 76 patients with DN were randomly divided into treatment group （39 cases） and control group （37 cases）. Conventional treatments were given to both groups. The control group received beinazepril, 10 mg/d, while the treatment group received blood - flow - promoting herbs at a dose of 4 pills, 3 times a day, for 12 months. Results There was no significant difference in the rates of urinary albumin execretory, β2 -- MG in serum, β2 -- MG in urine and α1 - MG in urine of two groups before the treatment （ P 〉 0. 05）, After the treatments, significant difference was found in UAER, β2 - MG in serum, β2 - MG and α - MG in urine, FIB, D - D between two groups （ P 〈0. 05） ; In the treatment group, FIB and D - D decreased significantly after treatments （ P 〈 0. 05）. There was no difference in FIB, D - D between pre - therapy and post - therapy in the control group （ P 〉 0. 05 ）. Conclusion The effects of blood - flow - promoting herbs are significantly better than that of beinazepril in reducing albuminuria, improving renal function and hypercoagulability in patients with DN.
