摘 要:目的探讨T淋巴细胞CD28表达与乙肝病毒携带(ASC)者免疫耐受的关系。方法采用流式细胞仪定量检测ASC者外周血及肝组织中CD28的水平;采用免疫组化技术检测ASC者肝组织CD28的表达率。结果不同Knodell积分的ASC者肝组织T淋巴细胞CD28表达情况间差别有显著性意义(P<0.05),ASC者中83.9%为无表达或低表达。ASC者外周血及肝组织中CD8+/CD2+8表达率分别为(20.0±5.8)%和(19.0±6.8)%,均较正常组低(P<0.05),且肝组织中CD8+/CD2+8水平较外周血下降更明显,差别有显著性意义(P<0.01)。肝组织中CD8+/CD2+8表达率随着肝组织学积分的减少而下降。结论ASC者肝组织CD28表达明显低下,T淋巴细胞不能完全活化,处于相对免疫耐受状态。ASC者肝组织中CD8+/CD2+8表达率较外周血更能反映肝脏免疫耐受状态。T淋巴细胞CD28表达与ASC者免疫耐受关系密切,且在其免疫耐受发病机制中起着重要作用。Objective To inquire into the relationship between CD28 of T - lymphocytes and immune tolerance in hepastitis B virus carriers. Methods Flow cytometry (FCM) was used to estimate quantitatively the changes of CD28 levels of peripheral blood and hepatic tissue in hepastitis B virus carriers; Expressions of CD28in hepatic tissue were determined by immuno - histochemical method. Results Significant difference was found in expression of CD28 of T - lymphocyte in hepatic tissus between different patients with different Knodell integrals of hepastitis B virus ( P 〈 0. 05 ) ; There was no expression or low expression in 83 9% of the patients ; The expression rates of CD8^+/CD28^+ in peripheral blood and hepatic tissues were respectively ( 20. 0 ± 5.8 ) % and ( 19. 0 ± 6. 8 ) %, lower than that of the normal group ( P 〈 0. 05 ) , and there was significant difference in levels of CD8^+/CD28^+ between hepatic tissues and peripheral ( P 〈0. 01 ) ; The expression of CD8^+/CD28^+ reduced with the decline of hepatic tissue integral. Condusion The expression of CD28 of hepatic tissues in patients with hepatitis B virus is significantly low, and T- lymphocyte can't be fully activated in a relative immune tolerance state. The expression of CD8^+/CD28^+ in hepatic tissues can reflect immune tolerance state more than that of peripheral blood. There is a close relation between expression of CD28 in T - lymphocyte and immune tolerance in patients with hepatitis B virus. Expression of CD28 in T - lymphocute plays an important role in immune tolerance pathologic mechanism.
