摘　　要：目的分析老年原发性高血压患者的颈动脉粥样硬化(CAS)与血尿酸(SUA)及相关炎症因子的关系。方法选取老年原发性高血压患者42例及血压正常者38例,应用高频多普勒超声检测颈动脉内膜-中层厚度(IMT)及斑块情况,测定血清三酰甘油(TG)、总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、SUA、超敏C反应蛋白(Hs-CRP)、白介素6(IL-6)和肿瘤坏死因子-α(TNF-α)。结果老年高血压组的SUA、TNF-α、IL-6、Hs-CRP、颈动脉IMT及CAS的发生率明显高于对照组,差异有统计学意义(P<0.05);颈动脉IMT与收缩压(SBP)、SUA、Hs-CRP呈正相关。结论老年高血压CAS的发生和发展除了血压增高的影响因素外,体内SUA的增高及相关炎症因子也参与了CAS的病理生理过程。Objective To investigate the relationship of serum uric acid （SUA） and inflammatory factors with carotid atherosclerosis（CAS） in elderly patients with essential hypertension（EH）. Methods All elderly subjects were examined by ultrasound to measure the intima-media thickness （IMT） of carotid artery and the extent of plaque formation. The subjects included 42 elderly EH patients and 38 control subjects. Serum triglyceride（ TG）, total cholesterol （TC）, low density lipoprotein cholesterol （LDL-C） , high density lipoprotein cholesterol（HDL-C） , SUA, high-sensitivity C-reactive protein （Hs-CRP） , intedeukin-6 （IL-6） and tumor necrosis factor-α （TNF-α） were measured. Results The levels of SUA,TNF-α,IL-6, Hs-CRP,the IMT of carotid artery and the incidence of CAS in EH group were significantly increased than those in control group （ P 〈 0. 05 ） . The levels of SBP, SUA, Hs-CRP had positive correlation with carotid IMT in elderly EH group. Conclusions In addition to increased blood pressure, increased serum uric acid and the corresponding inflammatory factors are strongly and closely related to the development of CAS in elderly patients with EH.
