摘  要:目的探讨爆炸性武器冲击波原发效应致伤颅脑后的CT诊断价值。方法将1gTNT当量的球型爆炸源置于距右颞顶部不同距离(5mm、9mm、13mm)对犬的颅脑部引爆致伤,制造动物模型,伤后即时、6h后、12h后、24h行头颅CT冠状扫描,观察CT表现。结果伤后即时扫描发现了额窦积血、脑挫裂伤、硬膜下出血、硬膜外血肿、颅内血肿、气颅及颅骨骨折等阳性表现,动态CT扫描追踪到了脑挫裂伤病灶的低密度范围扩大趋势。结论通过首次CT检查,可以确定颅脑爆炸伤的具体伤情,连续CT扫描可以追踪颅内血肿、脑挫裂伤病灶的动态变化。Objective To explore CY diagnostic value of the craniocerebral injury by the prirmary effect of the blast wave of explosive weapons. Methods A spherical explosive with explosive energy of I g TNT was used to detonate at different distances (5 nun, 9 nun, 13 nun) to cause the explosive injury of dog brain. The dog heads were performed coronal scan at 0 h, 12 h, 24 h after explosion. The CT characteristics were observed. Results Hematocele in frontal sinus, contusion and laceration of brain, subdural hemorrhage, epidural hematoma, intracranial hematoma, pneumocephalus and skull fracture were found by the immediate CT scan after wound. The enlarged tendency of low density circumscription of the contusion and laceration of brain were traced by the sequential CT scanning. Conclusion The extensional traumatic condition of the explosive injury of brain can be determined by the first CT scanning. The development of the intracranial hematoma and contusion and laceration of brain may be traced by sequential CT scanning.
