治疗急性左心功能衰竭中主动脉内球囊反搏术的应用和探讨

摘　　要：目的：应用主动脉内球囊反搏术治疗心脏手术后患者的心功能衰竭，本文旨在提高主动脉内球囊反搏术的使用成功率。方法：本文报告３４例采用Ｄａｔａｓｃｏｐｅ－８３型或９５型驱动装置和球囊反搏导管，１７例采用切开股动脉暴露法，１７例采用经皮穿刺股动脉插管法。结果：全组３４例中存活１７例，成功率为５０％；发生并发症６例，下肢缺血３例，血肿２例，右股动脉血栓形成１例。结论：对于急性左心功能衰竭患者及时合理地应用主动脉内球囊反搏术是一种有效的治疗方法。Objective:The article is to increase the successful rate of intra aortic balloon pump(IABP)in patients with acute pump failure after open heart surgery. Methods:This group included 34 cases using Datascope 83 or Datascope 95 pump.In 17 cases intra aortic balloon(IAB)catheters were inserted into excised femoral arteries through artifical blood vessels,while in the others IAB catheters were inserted percutaneously. Results:Seventeen patients survived.Among them,6 had complications,3 of ipsilateral ischemia of lower limb,2 of hematoma in insertion site and 1 of thromboembolism of right femoral artery.All of them recovered after surgical intervention. Conclusion:A timely reasonable application of IABP is an effective method in treating patients with acute pump faiure after open heart surgery.
