昆明地区正常新生儿及窒息儿神经元特异性烯醇化酶的临床研究

摘　　要：目的为建立昆明地区正常新生儿血清神经元特异性烯醇化酶(NSE)参考值范围提供依据;了解围产高危因素对NSE浓度的影响;NSE浓度变化与新生儿窒息发生率及严重程度的相关性。方法用免疫放射法对100例正常新生儿和100例存在围产高危因素新生儿的羊水、脐动脉血及生后3d,5d血清中NSE测定,对30例窒息儿生后1d,3d,5d及14d的血清进行NSE测定。结果正常组脐血、3d和5d血清NSE值分别为(22.2±10.9)ng·ml-1,(25.3±12.3)ng·ml-1和(18.3±8.7)ng·ml-1,三者间差异无显著性(P>0.05);正常、高危组均提示羊水NSE均数代表性差;围产高危因素组生后3d血清NSE均数比较该组与窒息组间的差异有显著性(P<0.05),与正常组间差异无显著性(P>0.05);围产高危因素中羊水污染组与窒息组及正常组间差异有显著性(P<0.05);窒息组NSE浓度高,病情好转随之下降,第1d与第5d,14d比较以及第3d与14d血清NSE均数间比较差异有显著性(P<0.05);窒息儿中67%NSE浓度高于正常组,而且窒息程度越重浓度越高。结论要建立昆明地区正常新生儿血清NSE参考值范围还应进行更大样本的调查;围产高危因素中羊水污染与窒息及缺血缺氧性脑损伤关系密切;NSE与缺血缺氧性脑损伤病程相符,与病情严重程度呈正相关;NSE与窒息发生率及窒息的严重程度有较密切的相关性。Objective To provide a basis for the establishment of normal reference values of the serum neuron-specific enolase （NSE） of Kunming healthy newborns ; to understand the impact of perinatal risk factors for NSE ; to understand the NSE concentration and the incidence and severity of neonatal asphyxia. Method Using immunoradiometric assay, the NSE in amniotic fluid and umbilical blood of 100 normal newborns and 100 cases of neonate with high risk factors were determined. The determinations of serum NSE in 30 neonates with asphyxia after 1 day, 3 day, 5 days and 14 days were also made. Results The differences between serum NSE of the three-day-old boys and girls were significant （P 〈 0.05） ; the difference between the neonate group with risk factors and asphyxia group was significant （P 〈 0.05） The NSE of newborns with asphyxia was at high concentrations, after the condition im- proved, it fell. In asphyxia newborns, the more severe asphyxia accompanied with higher concentration of NSE. Conclusion The normal reference values should be established in Kunming with larger sample survey.
