原发性亚甲减对记忆功能的影响及替代治疗的价值

摘　　要：目的评估成年原发性亚临床甲状腺功能减退症(亚甲减)患者的记忆功能及替代治疗的价值。方法运用修订韦氏记忆量表和N-back任务检测19例初诊成年原发性亚临床甲状腺功能减退症(SCH)患者、16例初诊甲状腺功能亢进症(甲亢)患者和18例正常对照者的记忆功能,其中10名SCH患者左旋甲状腺素替代治疗半年后重复上述检测。结果SCH组患者血清游离甲状腺素水平低于对照组(P<0.05)。修订韦氏记忆量表检测显示,SCH组记忆商较对照组低下(P<0.05),其中触觉和背数分项有统计学差异(P<0.05)。N-back任务检测显示,SCH组2-back任务正确率显著低于对照组(P<0.01)。SCH组替代治疗半年后血清促甲状腺素和甲状腺素水平均恢复正常,韦氏记忆量表和2-back任务检测结果较治疗前提高(P<0.05),与对照组无明显差异。甲亢患者记忆量表和N-back任务正确率与正常组无明显差异。结论亚甲减患者存在可逆性记忆功能损害。N-back任务可能是检测SCH患者记忆功能的较好工具。[Objective] To evaluate the memory function in adult with subclinical hypothyroidism（SCH）and the value of Levo-thyroxine replace treatment.[Methods] The memory function of 19 newly diagnosed SCH patients,16 newly diagnosed hyperthyroid patients and 18 euthyroid control subjects was detected by the Wechsler Memory Scale-Chinese Revision（WMS-CR） and N-back task.After treated with levo-thyroxine for 6 months,ten SCH patients were detected by the above–mentioned experiment.[Results] Serum free thyroxine level in SCH group was lower than that of control group（P〈0.05）.WMS-CR test showed that memory function of SCH group was lower than that of control group（P〈0.05）,especially in the subitem of touch and reback the number（p〈 0.05）,2-back task positive rate of SCH patients was significantly lower than that of control group（p〈0.01）.After replace treatment for 6 months,serum thyrotropin and thyrine levels in SCH group all revived normal.The results of WMS-CR and N-back task were higher than those before treatment（P〈0.05）,but had no difference compared with control group.There was no difference in WMS-CR and N-back task positive rate between control and SCH group.[Conclusion] The SCH patients may exist reversible memory impairment.N-back task may be a better means of detecting memory function in SCH patients.
