超声二维应变成像技术在评价扩张型心肌病患者左心收缩功能中的应用

摘　　要：目的应用超声斑点追踪成像技术评价扩张型心肌病(DCM)患者的左心收缩功能及左心扭转。方法获取31例DCM患者与29例正常人的标准心尖位左心流出道切面,四腔心切面,两腔心切面,短轴左心室二尖瓣水平、乳头肌水平、心尖水平的图像,应用超声斑点追踪技术测量各个室壁运动的收缩期最大应变,并测量二尖瓣水平及心尖水平心室的旋转角度。比较DCM组及正常组的收缩期最大应变和旋转角度。结果与正常人相比,DCM患者收缩期左心室心尖位室壁的最大纵向应变明显低于正常组(P<0.01)。短轴二尖瓣、乳头肌、心尖水平的径向应变、圆周应变、及旋转角度也明显低于正常人(P<0.01),且总体的旋转角度与患者的心功能成正相关(r=0.59,P=0.004)。结论DCM患者的左心收缩功能显著受损,超声斑点追踪技术可以科学、准确地反映左心室收缩功能,对DCM患者心功能的判断具有重要价值。Objective This study was performed to evaluate the systolic function and torsion of the left ventricle in patients with dilated cardiomyopathy by measurement of left ventricular strain in short-axis and long-axis using speckle tracking echocardiography. Methods Two dimensional images were recorded from the left ventricular short-axis view at the mitral annulus, papillary muscle and apical levels, and the apical four-chamber view, two-chamber view and long-axis of the left ventricle in 31 patients with dilated cardiomyopathy and 29 healthy subjects were recorded using GE Vivid 7 dimension and the 3S probe with 1.7-3.4 MHz. The peak systolic longitudinal strain were measured by speckle tracking echocardiography in apical views. Peak systolic circumferential strain, radial strain and rotation were measured in the left ventricular short-axis views using 2D strain software. Results When compared with healthy subjects, all parameters in dilated cardiomyopathy were statistically significantly lower than those in the healthy subjects （P〈0.01） . In patients, there was a good correlation between left ventricular ejection fraction and left ventricular systolic torsion. Conclusions Speckle tracking echocardiography can be used as a new method to evaluate the left ventricular systolic function quantitatively.
