自锁髓内钉治疗肱骨干骨折的临床疗效

摘　　要：目的总结肱骨自锁髓内钉治疗肱骨干骨折的临床疗效。方法对41例肱骨干骨折患者均行自锁髓内钉固定,其中男29例,女12例。年龄25～61岁,平均44岁。闭合性32例,开放性9例。闭合复位23例,切开复位18例。结果41例均得到随访,术后随访平均22个月(6～42个月)。41例骨折均愈合,骨折平均愈合时间为18周(10～32周)。肩关节功能按Neer评定法评价,优良率为95.1%。结论采用肱骨自锁髓内钉治疗肱骨干骨折操作简单,损伤小;轴向弹性固定,骨折愈合快;固定牢靠,有利于早期功能锻炼,肩关节功能影响小,疗效满意。Objective To investigate the surgical treatment of the humeral shaft fracture with intramedullary self-lock- ing nail. Methods 41cases with the humeral shaft fractue were treated with intramedullary self-locking nail.There were 29 males and 12 females whose age ranged frome 25 to 61 years old,averaged 44 years old. 32 cases were closed fractures, 9 cases were openeded fractures. 23cases were performed with closed implantation,the other were performed with opened implantation. Results All cases were followed up for 6 - 42months, 22months on average, and all showed bony union within 10 - 32weeks, 18weeks on average. The Neer method was used to assess the results of surgical treatment. 27cases gained the excellent results,12good and 2poor. Conclusions The surgical treatment of the humeral shaft fracture with intramedullary self-locking nai is effective, with simple operation, solid fixation. Due to the abutment of the longitudinal bars along the medullary canal,the time to fracture union can be decreaced.Functional exercise can be started at early stage.
