多普勒组织成像技术对高血压性左室肥厚舒缩功能的评价

摘  要:目的采用多普勒组织成像(DTI)技术对高血压左心室肥厚的患者左心室舒缩功能进行评价。方法26例高血压左心室肥厚患者(高心组)、20例正常人(对照组)。应用DTI技术测量各组室间隔及左室后壁收缩期,舒张早、晚期峰值运动速度(分别是Vs、Ve、Va)、内外膜峰值速度差(△V)及峰值速度阶差(VG)、Ve/Va比值、等容收缩期及等容舒张期心肌运动频谱持续时间(IVCT、IVRT),并进行比较。结果与对照组相比,高心组室间隔及左室后壁明显增厚,LVMI明显增大,E峰和E/A值明显减小(P<0.01),Ve、△Ve、VGe显著降低(P<0.01)。结论DTI技术可实时、准确地评价心肌舒缩功能状况。Objective This study was performed to assess the myocardial contractile and relaxant function in hypertension with left ventricular hypertrohy (LVH) in patients by Doppler tissue imaging(DTI). Methods In 20 normal subjects and 26 patients with hypertension and LVH,the motion of interventricular septum (IVS) and left ventricular posterior wall (LVPW) were assessed by DTI. Some parameters such as systolic peak velocity(Vs) ,early -diastolic and late -diastolic peak velocity (Ve, Va respectively), velocity difference ( ΔV ) and velocity gradient (VG) between endocardium and epicardium, Ve/Va ratio,isovolumic contraction time and isovolumic relaxation time (IVCT, !VRT respectively) were measured. Results Compared with control group, the wall thickness of IVS and LVPW and LVMI of LVH group was significantly increased, E peak velocity and ratio of E and A of LVH group were significantly decreased( P 〈0. 01 ). The Ve, AVe and VGe of the LVS in LVPW and IVS in LVH group were significantly lower than that of control group ( P 〈 0. 01 ). Conclusion The DTI can be used to assess the function of left ventricular quickly and sensitively.
