机采血小板在妇科大型手术中的应用

摘　　要：目的分析妇科大型复杂手术中采用预防性输注机采血小板的效果。方法选择妇科大型复杂手术病例共46例,随机分为观察组和对照组各23例,观察组除在术中常规输注晶胶体液外,手术开始0.5h预防性输注机采血小板1个治疗剂量单位,对照组则按常规作术前准备。结果两组的手术时间、术中出血、术中输血量和住院天数的差异均有显著性(P<0.01),观察组手术时间明显缩短,出血量显著减少,异体输血量明显降低,住院时间低于对照组6～8d。结论妇科大型复杂手术中预防性输注机采血小板能减少术中出血量,缩短手术时间,减少术中输血,提高了手术质量。Objective To observe the haemorrhage in gynecological large-scale operations after transfusing 1 unit platelet. Methods 46 cases of gynecological large-scale operations were randomly divided into two groups：23 cases in experimental group were transfused not only peptizer and normal saline solution but also 1 unit platelet at the first half an hour of the operation prevently, 23 cases in control group were transfused peptizer, normal saline solution and transfused RBC 2-8 unit according to the amount of bleeding. Results The amounts of bleeding and the blood transfusion throughout the operation in experimental group were less than those in the control and the time of operation was shorter than that in the cortrol group significantly（P〈0. 01）. And the time in hospital was abated about 6-8 days. Conclusions Transfusing platelet prevently in gynecological large-scale operations could reduce the haemorrhage and the blood transfusion, short the operation time. Furthermore ,it was favor in elevating time quality of operation and benefit for the patient.
