摘 要:目的将高磷酰胆碱基团接枝的壳聚糖(PC-Chi)涂在微弹簧圈(MC)上,研究评价应用多孔复合膜微弹簧圈骨架系统(PC-Chi-MC)栓塞动脉瘤的治疗效果。方法体外研究PC-Chi与人脐静脉内皮细胞(HUVEC)的细胞相容性,以平行板流动装置使切应力分别为5和40 Pa,流动作用时间24 h,测定流体切应力对PC-Chi的HUVEC黏附性的影响;将Chi、PC-Chi分别涂层在MC的表面,以强力拉伸机测定多孔复合膜MC骨架系统的拉伸强度和弹性模量。将MC和PC-Chi-MC分别栓塞在侧壁型动脉瘤腔中,3个月后观察内皮细胞覆盖动脉瘤颈的情况。结果PC-Chi与HUVEC的细胞相容性很好,细胞增殖超过了普通细胞培养板的性能(67.43%);在40 Pa切应力条件下PC-Chi表面培养内皮细胞24 h,细胞的形态尚未发生明显改变,20 mm以下位移时PC-Chi-MC与MC的应力变化差异无统计学意义(P>0.05);以Pc-Chi-MC栓塞的动脉瘤瘤颈处有较多的内皮细胞覆盖,表现出良好的治疗效果。结论应用PC-Chi-MC栓塞AN可以使内皮细胞尽早在瘤颈部位生长、愈合,这种情况有可能减少栓塞后动脉瘤的再通、复发,从而增强动脉瘤栓塞后的长期疗效。Objective To coat phosphorylcholine-substituted chitosans (PC-Chi) on microcoil (MC) and evaluate the efficacy of endovascular embolization in aneurysms with micro-porous composite membrane coil skeleton system (PC-Chi-MC). Methods The biocompatibility of PC-Chi was measured by in vitro cultured with HUVEC. Using apparatus which the shear stress level respectively confined to 5 and 40 Pa for 24 hours, and study the response of cultured HUVEC to fluid shear stress ; PC and PC-Chi were coated on MC. Tensile apparatus was used to evaluate mechanical properties of PC-Chi-MC such as tensile strength and elastic modulus; Laterial saecular aneurysms were embolized with MC and PC-Chi- MC. Three months later the endothelial cell coverage of the aneurysm neck was observed. Results PC-Chi showed good biocompatibility, HUVECs proliferation on tissue culture treated PC-Chi were better than on the ordinary cell culture plate (67.43 % ) ;On 40 Pa shear stress condition for 24 hours, the morphology of endothelial cells on culture treated PC-Chi had not significantly changed ; Below 20mm displacements, there was no significant change of the stress difference between PC-Chi-MC and MC ( P 〉 0.05 ). PC-Chi-MC embolization show more endothelial cells coverage of the aneurysm neck. Conclusion Embolization with PC-Chi-MC can help endothelial cells to grow and heal in aneurysm neck as soon as possible. This may help reduce recanalization and relapse after aneurysm embolization, thus enhancing the long-term effect of aneurysm embolization.
